摘 要:目的比较野战综合手术救治方舱与导管室内介入治疗影像质量和辐射剂量的差异。方法用标准体模分别测量影像清晰时(dose-area product,DAP)、入射皮肤剂量(enter skin dose,ESD)透视和摄影的辐射剂量值,对6只实验动物分别在野战救治方舱和导管室内行冠状动脉造影,由3位观察者采用双盲法对影像质量进行比较分析。结果方舱内DAP透视和摄影时的剂量值分别为(12.96±0.19)μGym2/s,(39.55±0.26)μGym2(10帧);ESD依次为(21 403.33±170.10)μGy/s,(67 647.25±173.12)μGy(10帧);导管室内DAP透视和摄影时的剂量值分别为(5.20±0.01)μGym2/s,(55.27±4.32)μGym2(10帧);ESD依次为(11 690.00±26.46)μGy/s,(106 430.82±49.57)μGy(10帧)。方舱组和导管室组剂量比较均有统计学差异(P<0.05),两组影像质量无统计学差异(P>0.05)。结论野战综合手术救治方舱相比导管室,可以降低辐射剂量且影像质量满足诊断要求。Objective To compare the difference of image quality and radiation dose between field shelter and cardiac catheterization room during intervention therapy.Methods The dose-area product(DAP) and entrance skin dose(ESD) were measured respectively with standard phantom in fluoroscopy and digital acquisition.Six laboratory animals were separately operated on intervention therapy in field shelter and cardiac catheterization room.Three observers used double-blind method to evaluate the digital subtraction angiography(DSA) image.Results The DAP valve of field shelter in fluoroscopy and digital acquisition were(12.96±0.19) μGym2/s and(39.55±0.26) uGym2 respectively(10 image plus test exposure).And ESD were(21 403.33±170.10) μGy/s and(67 647.25±173.12) μGy respectively(10 image plus test exposure).The DAP valve of cardiac catheterization room in fluoroscopy and digital acquisition were(5.20±0.01) μGym2/s and(55.27±4.32) μGym2 respectively(10 image plus test exposure),And ESD were(11 690.00±26.46) μGy/s and(106 430.82±49.57)μGy respectively(10 image plus test exposure).The dose was significantly different between two groups(P0.05),and the image quality was not different(P0.05).Conclusion Intervention therapy in field shelter can reduce the radiation dose compared with catheterization room and guarantee the image quality for diagnosis.
