虎口背侧皮瓣修复手部皮肤软组织缺损应用临床分析

摘　　要：目的总结虎口背侧皮瓣修复手部皮肤软组织缺损的临床价值与经验体会。方法回顾性分析2010年1月～2011年12月67例虎口背侧皮瓣修复手部皮肤软组织缺损患者的相关临床资料。结果本组病例的术后住院时间为8～26d,平均(14.9±1.3)d。术后随访3～8个月,平均(4.9±0.6)个月。本组病例中,皮瓣Ⅰ期愈合63例(94.0%),皮瓣的外形及供区功能较为满意,皮瓣色泽、弹性、质地、感觉等恢复良好。其余病例中,术后出现皮瓣张力性水泡3例(4.5%),予拆线后成活;桡背侧动脉皮瓣远端回流障碍1例(1.5%),部分拆开蒂部缝线后,给予肝素盐水湿敷,术后23d愈合。结论虎口背侧皮瓣修复手部皮肤软组织缺损具有操作简单、成活率较高及术后效果良好等优点,值得进一步推广和应用。Objective Summary of first dorsal skin flap repair of soft tissue defect of hand and experience of clinical value. Methods A retrospective analysis of our hospital from 2010 January -2011 year in December 67 cases of dorsal hand flap to repair hand skin soft tissue defect in patients with clinical data. Results In the cases of postoperative hospitalization time was 8-26 d, the average （14.9±1.3） d. After follow-up for 3-8 months, mean （4.9±0.6） months. In this group of cases, the flap healed in 63 cases （94.0%）, flap of the appearance and function of donor area is satisfied, elasticity, texture, color, skin feel, good recovery. The remaining cases, postoperative flap tension vesicle in 3 cases （4.5%）, to the stitches after survival; dorsal radial artery flap for distal reflux disorders in 1 cases （1.5%）, portions apart of pedicle of sutures, given heparin saline, 23 d postoperative healing. Conclusion Its dorsal skin flap in the repair of hand skin soft tissue defect has the advantages of simple operation, high survival rate and postoperative effect of good, worthy of further promotion and the application.
