定量组织速度成像及彩色M型超声对心房颤动患者左心室功能评价

摘　　要：目的应用超声心动图技术评价高血压性心房颤动(hypertension atrial fibrillation,HAF)及孤立性心房颤动(lone atrialfibrillation,LAF)患者的左心室收缩、舒张功能。方法将50例心房颤动患者,分为孤立性房颤组,高血压性房颤组,分别采集房颤患者的心室率、左房左室结构参数、二尖瓣血流频谱、彩色M型及二尖瓣环的组织多普勒运动曲线上的参数,并与正常对照组比较。结果较正常对照组,房颤组心室率、左房结构参数、二尖瓣血流频谱及彩色M型、二尖瓣环组织多普勒运动曲线上的参数差异有统计学意义(p<0.05),较孤立性房颤组,高血压性房颤组彩色M型、二尖瓣环组织多普勒运动曲线上的参数差异有统计学意义(p<0.05)。结论 (1)房颤组患者在左室结构未改变时,左房增大,其左室收缩、舒张功能已受损;高血压房颤组较孤立性房颤组左室功能损害更明显;(2)组织速度成像、彩色M型能敏感、快速、准确地定量分析房颤患者的心功能。Objective To evaluate the left ventricular （LV） function in patients with HAF and LAF by quantita- tive Tissue Velocity Imaging （QTVI） and Color M-mode. Methods The patients with AF were divided into the groupwith LAF and the group with HAF. Parameters derived frum the ventricular rate, rouline 2D uhralsound, transmitral flow spectral , color M-mode and ClUantita live tissue velocity imaging （QTVI） were acquired in the two groups and compared with the normal group. Results Partial parameters derived from the ventricular rate, routine 2D ultralsound of I.A, transmitral flow spectral, color M mode and quantitative tissue velocity imaging （QTVI） were significantly different between the normal group and the group with AF （P〈0.05）.Partial parameters derived from the color M mode and quantitative tissue veloeity imaging （QTVI） were significantly different between the patients with LAF and the patients with HAF （P 〈0.05）. Conclusions There are Left atrial enlargement and diastolic （and systolic） dysfunction of LV in the group with AF when left vcntricular structure have not changed. Compared with the patients with LAF,the LV function were significantly impaired in patients with HAF, Parameters derived from the transmittal flow spectral, QTVI and color M mode can quantify the LV and LA function in AF patients sensitively, rapidly and ac- curately.
