高原高寒地区突发事件院前急救转运中的护理组织管理

摘　　要：目的探讨护理组织管理在高原高寒地区危重患者院前急救转运中的应用效果。方法以2008年6月～2010年5月转入兰州军区兰州总医院的145例危重患者为对照组,以2010年6月～2012年5月转入兰州军区兰州总医院的133例危重患者为观察组,对照组行常规的院前转运护理,对观察组在对照组常规护理措施基础上实施了一套完整的护理组织管理;比较两组在急救转运时的意外发生率、死亡率、患者及其家属的投诉率,并评价兰州军区兰州总医院实施护理组织管理措施的效果。结果观察组患者意外情况发生率及死亡率、护理投诉率依此为9.0%、0.8%、5.3%,分别低于对照组的33.1%、7.6%、17.9%,经比较均具有统计学意义(χ2=17.360、5.701、8.303,均P<0.05)。结论加强护理组织管理,提升出诊护士的专业技能、应变应急能力及处理风险的能力,可提高高原高寒地区危重患者院前急救转运的安全性。Objective To investigate the application effect of care organization and management in critically ill patients with pre-hospital emergency transport in high altitude alpine region. Methods 145 cases of critically ill patients transferred to our hospital from June 2008 to May 2010 were taken as the control group, 133 cases of critically ill patients transferred to our hospital from June 2010 to May 2012 were taken as the observation group. The control group underwent conventional hospital care before transporter. The observation group implemented a complete care organization and management on the basis of the usual care measures of control group. The rate of emergency transport accident, mortality rate and complaint rate were compared, and the effect of care was evaluated in our hospital. Results The rate of emergency transport accident, mortality rate and complaint rate in observation group were 9.0%, 0.8% and 5.3%, respectively, lower than 33.1%, 7.6%, 17.9% of the control group, the comparison between the two groups were statistically significant （χ2=17.360, 5.701, 8.303, P 〈 0.05）. Conclusion Intensive care of organization and management, to enhance the professional skills of the visiting nurse, strain the capacity of the emergency response capacity and processing risk, can improve security of pre-hospital emergency transport for critically ill patients in plateau alpine region.
