摘　　要：目的系统评估生酮饮食治疗难治性癫痫的研究进展。方法以“生酮饮食、癫痫、难治性癫痫、饮食疗法等”为主题词，通过计算机检索包括中国学术文献总库、重庆维普中文科技期刊数据库、万方期刊及学位论文数据库、中国生物医学文献数据库（CBM）、EMBASE、PubMed、以及Cochrane图书馆临床对照试验数据库，并辅以文献追溯、手工检索等方法，尽可能收集2011年1月至2012年12月国内外公开发表的文章及学位论文中关于生酮饮食治疗难治性癫痫的临床资料，并追查所有纳入研究的参考文献。按纳入排除标准纳入合格的随机对照研究并对其进行质量评价，Meta分析采用Revman5．1软件进行。结果共纳入文献11篇，共计627例，其中包含5篇随机对照研究，1篇回顾性研究，5篇前瞻性研究。结论Atkins饮食及改良的饮食方案治疗难治性癫痫耐受性较好，对儿科临床具有一定的指导意义。此外，我们可以在生酮饮食治疗过程中密切监测儿童血浆中维生素、矿物质等微量营养素的变化，以便于缺失时能够得到及时补充。Objective To systematically review the evidence from the included trials regarding the research progress of ketogenic diet for intractable epilepsy .Methods We used certain MeSH such as ketogenic diet, epilepsy, intractable/refractory epilepsy, dietary therapy etc. to research from Academic literature database, Chongqing Chinese science and Technology Periodical Database, Wan fang journals and Academic degree dissertation database, Chinese Biomedical Literature, Database（CBM）, EMBASE, PubMed, and Cochrane Library Clinical controlled trial database by the computer, at the same time, we made the fully use of the literature review and manual search. We tried our best to search all literatures on ketogenic diet for intractable epilepsy from articles and Degree thesises which published at home and abroad and aged from January 2011 to December 2012,and traced all the references included in this study. We assessed the quality of qualified randomised controlled studies according to the inclusion criterion. Then we performed the meta-analysis by Rev Man 5. l.Results We included 11 articles altogether. The studies recruited a total of 627patients. No age restrictions were imposed. Five randomised controlled trials, one retrospective studies and five prospective studies were identified. Conclusion The tolerability is good for patients with medically intractable epilepsy treated with the Modified Atkins Diet and other modified diets. It guided for the pediatric clinical. Furthermore, we may monitor the changes of plasma vitamins, minerals during the ketogenic diet therapy in order to supplement them in time.
