下呼吸道感染病原菌分布及耐药性分析

摘  要:目的了解下呼吸道感染常见病原菌分布及其耐药性,为临床合理使用抗菌药物提供依据。方法对我院下呼吸道感染的住院患者痰标本中分离的病原菌进行鉴定及药物敏感性试验,并用WHONET5.5软件进行统计分析。结果共分离到1136株病原菌其中以革兰阴性杆菌为主共929株占81.8%,革兰阳性球菌共207株占18.2%。革兰阴性菌中以肺炎克雷伯菌265株(23.3%)、铜绿假单胞菌228株(20.1%)、鲍曼不动杆菌112株(9.9%)为主。其中肺炎克雷伯菌对亚安胺培南、美罗培南的敏感性均为100%;铜绿假单胞菌对头孢他啶、美罗培南、头孢吡肟较为敏感,其敏感性分别为89.9%、88.6%、88.1%;鲍曼不动杆菌对米诺环素、美罗培南较为敏感,其敏感性分别为82.5%、69.4%。革兰阳性菌中以金黄色葡萄球菌为主103株(9.1%),对万古霉素、替考拉宁、利奈唑胺敏感性均为100%。结论下呼吸道病原菌以革兰阴性菌为主,不同细菌中对同一抗菌药物敏感差异较大,应根据药敏试验结果合理使用抗菌药物。Objective To investigate the distribution of bacteria isolated from lower respiratory tract infection in order to provide reference for the clinical application of drugs. Methodes The strains of pathogenic bacteria were detected and their sensitivity to antibiotics was determined ,then the deta was analyzed by WHONETS.5. Resulter All 1136 pathogenic bacteria strain were detected, 929 (81.8%) were gram-negative bacilli, 207 (18.2%) were gram-positive cocci. The top 3 gram negative species were K.pneumoniae 265(23.3%) P aeruginosa 228 (20.1%) ,A.baumannii 112 (9.9%) .Imipenem and meropenem susceptive rate were 100% for K.pneumoniae. ceftazidime (89.9%)meropenem(88.6%) cefepime (88.1%) were the most active agents against P aeruginosa, minocycline (82.5%)meropenem (69.4%) were the most active agents against A.baumannii. The most of S. aureus was in Gram-positive cocci ,they were all sensitive to vancomycin ,Teicomycin Linezolid Conclusoin Clinical isolates are most of gram negative bacilli ,but the same Antimicrobial agent is different effective for difference bacteria. So it is very important to rationally use drug for clinical treatment of the infection according to the results of antibiotics susceptibility tests.
