改良模具在乙状结肠代阴道成形术中的临床应用

摘　　要：目的探讨术后采用双腔导尿管自制改良模具在乙状结肠阴道成形术中的临床应用价值。方法回顾性分析本院1995年1月至2011年12月收治的因先天性阴道缺如拟行乙状结肠阴道成形术,术中采双腔导尿管自制的改良模具的15例患者为研究对象。其中,有性生活患者为6例,无性生活为9例。术后对其均采用双腔导尿管自制的改良模具进行阴道扩张冲洗。对患者的阴道形成良好率、明显疼痛感、使用医从性、性生活满意度及相关并发症等进行分析(本研究遵循的程序符合本院人体试验委员会所制定的伦理学标准,得到该委员会批准,并与受试者签署临床研究知情同意书)。结果 15例(100.0%)患者乙状结肠阴道成形术及双腔导尿管自制改良模型的制作均成功。对其术后随访6～12个月,15例中14例(93.3%)坚持使用双腔导尿管自制的改良模具,阴道形成均良好,使用医从性好,仅1例(6.7%)因未坚持采用导尿管冲洗扩张而出现阴道中段狭窄。15例(100.0%)患者使用双腔导尿管自制的改良模具时均无明显疼痛感,并且无相关并发症发生。6例(100.0%)有性生活患者随访期间性生活均满意。结论采用双腔导尿管自制的改良模具取材方便、价格低廉、无需特殊制作即可使用,患者术后佩戴无痛苦,使用医从性高,术后无相关并发症发生,有一定的临床应用价值。Objective To evaluate the clinical value of improved mold modified by double channel catheter in sigmoid colon vaginoplasty. Methods From January 1995 to December 2011, a total of 15 patients with congenital absence of vagina were treated by sigmoid colon vaginoplasty in our hospital. Among them, only 6 cases had sexual life. Improved molds modified by double channel catheter were used to dilate and washing vagina in all cases after operation. Rates of excellent vaginal formation, postoperative analgesia, good compliance, satisfaction of sexual life and related complications were analyzed. The study protocol was approved by the Ethical Review Board of Investigation in Human Being of First People＇s Hospital of Zhaoqing City. Informed consent was obtained from all participates. Results Sigmoid colon vaginoplasty and improved mold modified by double channel catheter were all successfully in 15 cases （100.0%）. Six to twelve month follow-up showed that, there were 14 cases （93.3%, 14/15） insisted on using mold modified by double channel catheter with good rates of vaginal formation and good compliance, Only 1 case （6.7%, 1/15） who could not insist on using mold appeared vagina stenosis in middle piece. All 15 cases （100.0%） used improved mold modified by double channel catheter without obvious pain and no complications. Satisfaction of sexual life in 6 cases with sexual life after the operation were 100.0%. Conclusions Using the improved mold modified by double channel catheter in sigmoid colon vaginoplasty are easily derived without special-made and inexpensive materials. Patients use the improved mold had good compliance, and less complications. So it has certain clinical value.
