摘  要:目的通过测定糖尿病患者血浆单核细胞趋化蛋白-1(MCP-1)、8异前列腺素F2a(8-iso-PGF2α)、同型半胱氨酸(Hcy)、脂蛋白-α[Lp(α)]的水平,探讨其与左室舒张功能障碍的无症状糖尿病心肌病的关系及临床意义。方法 2型糖尿病(T2-DM)组125例(其中无症状心肌病组75例,单纯T2-DM组50例)及健康对照组30例。采用ELISA测定血浆MCP-1、8-iso-PGF2α、Hcy、氨基末端脑钠肽前体(NT-pro-BNP)、免疫比浊法测定血清Lp(α)和心脏多普勒超声测定室间隔厚度(IVST),比较各组的MCP-1、8-iso-PGF2α、NT-pro-BNP、Hcy、Lp(α)、IVST水平,并进行pearson相关性分析和多元线性逐步回归分析。结果糖尿病心肌病组的MCP-1、8-iso-PGF2α、NT-pro-BNP、Hcy、Lp(α)、IVST水平均高于单纯T2-DM组和正常对照组,差异有显著性(P<0.01);MCP-1、8-iso-PGF2α、Hcy、Lp(α)分别与NT-pro-BNP、IVST呈正相关(r=0.942,0.88,0.857,0.88,P=0.000;r=0.815,0.75,0.826,0.864,P=0.000);MCP-1、8-iso-PGF2α是NT-pro-BNP的主要影响因素,Hcy、Lp(α)是IVST的主要影响因素。结论 MCP-1、8-iso-PGF2α、Hcy、Lp(α)可能共同参与了糖尿病心肌病的发生发展。[ Objective ] To investigate the association of plasma monocyte chemoattratant protein-1 (MCP-1), 8- iso-prostaglandin F2a (8-iso-PGF2c~), homocysteine (Hcy), lipoprotein(cL) [Lp(ct)] and diabetic cardiomyopathy of left ventricular diastolic dysfunction in asymptomatic patients with type 2 diabetes mellitus. [ Methods ] Plasma mono- cyte chemoattractant protein-l, 8-iso-prostaglandin F2a, homocysteine, N-terminal-pro-brain natriuretic peptide (NT-pro-BNP), lipoprotein (o~) and interventricular septal thickness (IVST) in the diabetic eardiomyopathy of left ventricular diastolic dysfunction in asymptomatic patients with type 2 diabetes mellitus (n =75), type 2 diabetes with no cardiopathy (n =50) and the healthy persons (n =30) were determined, the level and correlation of MCP-1, 8-iso- PGF2ct, NT-pro-BNP, Hcy, Lp (ct) and IVST in type 2 diabetes and healthy persons were compared. [Results] Plasma MCP-1, 8-iso-PGF2ct, NT-pro-BNP, Hey, Lp (ct) and IVST levels of type 2 diabetic persons increased with statistical significance compared with the hea]thy persons. There was positive relevance between MCP-1, 8-iso- PGF2ct, Hcy, Lp (ct) respectively and NT-pro-BNP, IVST (r =0.942, 0.88, 0.857, 0.88, P =0.000; r =0.815, 0.75, 0.826, 0.864, P =0.000). MCP-1 and 8-iso-PGF2ct were risk factors of NT-pro-BNP. Hcy and Lp (ct) were risk factors of IVST. [ Conclusions ] Monocyte chemoattractant protein-I, 8-iso-prostaglandin F2a, homocysteine and lipoprotein (o~) might be possibly associated with the pathogenesis of diabetic cardiomyopathy.
