摘  要:目的:观察加巴喷丁联合吗啡镇痛对坐骨神经轴索切断大鼠脊髓神经丝蛋白(NF200)表达的影响。方法:选择重量在180~220g的成年雄性SD大鼠24只,随机分为四组(n=6):假手术组(S组),模型组(M组),术前开始镇痛组(P组)和常规镇痛组(N组)。M组、P组和N组建立左侧坐骨神经轴索切断模型,P组在术前1d至手术后9d使用加巴喷丁联合吗啡镇痛,N组在手术当日至手术后第9天给予加巴喷丁联合吗啡镇痛,S组和M组仅给予生理盐水。分别于手术前1d和手术后第3、5、7、9天测定各组右后足50%机械刺激缩足阈值,手术后第10天取腰5节段脊髓分别行改良三色法染色和神经丝蛋白(NF200)免疫组化染色。结果:M组在手术后各天50%机械痛阈均较S组显著降低,而P、N组均恢复正常水平;M组神经三色染可见部分髓鞘彻底溃变,Aβ纤维发生位置变化,而N组和P组神经三色染较M组有一定的缓解;免疫组化结果显示M组NF200表达较S组显著增高,而P组和N组较M组明显降低。结论:加巴喷丁联合吗啡对坐骨神经轴索切断大鼠有良好的镇痛效果,同时可改善坐骨神经轴索切断后相应脊髓节段的解剖学变化和NF200的表达变化。Objective To observe the effects of gabapentin combined with morphine on the expression of neurofilament protein 200 (NF200) in spinal dorsal horn in rats with unilateral axotomy. Methods 24 male Sprague- Dawley rats were randomly divided into sham operation group (S group), model group (M group), preventive analgesia group (P group), and normal analgesia group (N group). And the left common sciatic nerve was transected in M, P, N group. Gabapentin combined with morphine were given from pre-operative day 1 to postoperative day 9 to rats in P group, and were given from operative day to postoperative day 9 to rats in N group. Rats in M group and S group were given normal saline. 50% mechanical withdraw threshold of right hind paw were measured using Von Frey hair on pre-operative day 1 and postoperative day 3, 5, 7, 9 respectively. On postoperative day 10, morphologic changes of L5 spinal cord were evaluated by Meyer's triehrome staining and NF200 immunohistochemistry staining. Results Compared with S group, 50% mechanical withdraw threshold was decreased seriously in M group and was recovered to normal in P and N group. In M group, A-13 fibers sprouted into the superficial spinal dorsal horn in trichrome staining and this morphologic change was reduced in P and N group. The immunohistochemical expression of NF200 was increased in M group than those in P, N, S group. Conclusion Gabapentin combined with morphine could relieve the pain in sciatic nerve axotomy rats, improve the morphologic changes, and attenuate the increased expression of NF200 in spinal cord.
