摘  要:目的:评价扩散加权成像(diffusion weighted imaging,DWI)及其ADC值对肺内富血供结节良、恶性鉴别诊断中的应用价值。方法:收集在2007年10月至2010年12月期间行MR扫描且临床资料完整的孤立性肺结节病例45例,所有患者MRI检查包括常规MRI、DCE-MR、DWI。结果:在b=500s/mm2时,具备富血供特点的良、恶性结节之间ADC值有显著统计学差异(P=0.03)。把ADC值<0.00240mm2/s作为恶性肺结节的诊断依据时,其诊断灵敏度、特异度、阳性预测值、阴性预测值、正确指数、诊断符合率分别是82.61%、66.67%、86.36%、60%、0.493、78.12%,其中灵敏度和阳性预测值均非常高。结论:DWI检查有助于对临床上难以区分的富血供肺部结节良、恶性的鉴别。Objective To evaluate the application value of diffusion weighted imaging (DWI) and its parameter ADC values in the differentiation between benign and malignant pulmonary nodules with abundant blood supply. Methods From 2007 Oct to 2010 Dec, 45 consecutive patients were prospectively enrolled in the study. All patients underwent DCE-MR and DWI on a 1.5-T MRI unit. Results At b=500 s/mm2, significant differences were observed for ADC values between benign and malignant solitary pulmonary nodules with abundant blood supply (P = 0.03). When malignant nodules were diagnosed in the light of ADC value 〈 0.00 24 mm2/s, sensitivity, specifity, positive predictive value, negative predictive value, right index and diagnostic coincidence rate were 82.61%, 66.67%, 86.36%, 60%, 0.493 and 78.12%, respectively. Conclusion DWI could help to differentiate between benign and malignant pulmonary nodules with abundant blood supply which was difficult to define in clinic.
