腹部按摩联合热敷促进早产儿胎粪排出的效果观察

摘　　要：目的:观察腹部按摩联合热敷对促进早产儿胎粪排出的效果。方法:将100例早产儿随机分成试验组和对照组,每组50例。对照组早产儿主要靠自身肠蠕动来排胎粪,当患儿出现腹胀明显或胎粪排出延迟时使用开塞露纳肛的方法解决便秘和促进胎粪排出;试验组在早产儿生后6 h内开始给予腹部按摩及热敷,直至胎粪颜色由墨绿色转为全部黄色为止。观察两组早产儿首次排出胎粪的时间、首次排出胎粪的量、胎粪排尽的时间及腹胀发生率。结果:试验组早产儿首次排胎粪的时间明显提前,胎粪排尽的时间缩短,首次排出胎粪的量明显增多,腹胀发生率为0,两组比较差异具有统计学意义(P<0.05)。结论:腹部按摩联合热敷可促进早产儿胎粪排出,促进胃肠功能的恢复,并提高患儿舒适度,减少并发症的发生。Objective：To observe the effect of abdominal massage and hot compress for premature infant with meconium discharge. Methods： 100 eases of premature infants were randomly divided into the experimental group and the control group,50 cases in each group. The control group infants rely mainly on their intestinal peristalsis to meconium row, when infants abdominal distention were significantly or delayed passage of meconium using glycerine enema ad- mitted to anus methods to solve constipation and promoting meconium discharge. Experimental group in the proterm infants within 6 h after abdominal mas- sage and hot compress began to give,until the colour from dark green to meconium all yellow. To observe two groups of premature first meconium row time, first from meconium amount,meconium exhausting time and the incidence of abdominal distension. Results：The experimental group of preterm infants com- pared with the control group, the first meconium row ahead of time obviously, meconium exhausting time shortened, the first discharge of meconium in- creased, abdominal distension occurring at a rate was 0, the difference was statistically significant （P 〈 0.05 ）. Conclusion：Abdominal massage combined with hot compress can promote preterm meconium discharged, promote the recovery of gastrointestinal function, and increas the comfort,reduce the incidence of complications.
