血液分析仪XE-2100低值血小板标本血小板计数方法学比较

摘　　要：目的评价血小板(PLT)低值患者采用血液分析仪XE-2100电阻抗法(PLT-I)、光学法(PLT-O)两种方法检测血小板的准确性。方法将110例血液标本分成3组,PLT<50×109/L组、50×109/L≤PLT≥100×109/L组和PLT>100×109/L组。用电阻抗法(PLT-I)、光学法(PLT-O)和手工法同时计数血小板,以手工法计数为参考方法,分别与电阻抗法和光学法的检测结果进行配对t检验和相关性分析。结果 XE-2100仪器上,PLT>100×109/L光学法(PLT-O)和电阻法(PLT-I)计数血小板数量与显微镜计数数量比较,差异无统计学意义(P>0.05);血小板数量异常用光学(PLT-O)法与显微镜法比较,差异无统计学意义(P>0.05),而电阻法(PLT-I)计数与显微镜比较,差异有统计学意义(P<0.05)。在相关性上,PLT>100×109/L时电阻抗法、光学法与手工法均高度相关,PLT<100×109/L的标本,电阻抗法与手工法的相关性偏低,不及XE-2100的核酸染色技术与激光法结合血小板计数。结论 XE-2100血小板计数准确性较高,在大批量检测血液标本时可以使用电阻抗法检测,当血小板计数结果出现异常时应选择光学法进行检测,但在中小型医院,异常血小板计数还是首选人工镜检的方法。Objective To evaluate the accuracy of platelet （PLT） count tested by PLT - I and PLT - O with XE-2100 blood analyzer for the patients with lower platelet. Methods Blood samples of 110 cases were randomly divided into three groups ： PLT 〈 50 × 10^9/L, 50 × 10^9/L≤PLT≤100 × 10^9/L and PLT 〉 100 × 10^9/L. Patients with low value of platelet were determined with PLT - I and PLT-O methods, and microscopic method was used as the reference. The data were analyzed by test and correlation analysis. Results There was no statistical significance between PLT - I and PLT - O assays with PLT 〉 100 × 10^9/L （P 〉 0.05）. In the patients with abnormal platelet count, the PLT count determined by the methods of microscopy and PLT - O showed no statistically significance （ P 〉 0.05 ）, but there was statistically significance between the method of PLT - I and microscopy （ P 〈 0.05 ）. There was a high corre-lation among microscopic, PLT - I and PLT - O assay when the PLT was above100 × 10^9/L. Compared with nucleic acid staining tech- niques and laser method combined with platelet count, there was a low correlation between microscopic and PLT - I assay when the PLT 〈 100 × 10^9/L. Conclusion There is higher accuracy of the platelet count detected by XE - 2100. Assay can detect blood specimens in large quantities by using PLT-I, PLT-O assay should be chosen when the platelet count results appear abnormal detection.
