摘　　要：目的探讨N末端B型钠尿肽原(NT-proBNP)在急性心肌梗死(AMI)所致的不同级别心力衰竭中的临床意义。方法收集该院2009～2012年临床确诊的AMI伴心力衰竭患者90例,其中Ⅱ级28例、Ⅲ级38例、Ⅳ级24例。对照组为109例心功能正常者。测定患者血NT-proBNP,比较NT-proBNP在不同级别(Ⅱ、Ⅲ、Ⅳ级)的心力衰竭中的水平。结果 NT-proBNP在AMI伴心力衰竭Ⅱ级患者中为(1 328±847)pg/mL、Ⅲ级水平为(4 887±1 191)pg/mL、Ⅳ级水平为(26 605±5 636)pg/mL,每一级别都较对照组明显升高(P<0.05)。不同心力衰竭级别间差异有统计学意义(P<0.05),心力衰竭级别越高,其水平越高。38例在治疗后,随着心功能改善NT-proBNP显著降低(P<0.05)。结论 NT-proBNP在AMI患者中可作为心力衰竭指标,其水平对心功能的评价和判断预后有重要临床价值。Objective To investigate the clinical significance of N terminal B type natriuretic peptide（NT-proBNP）in different level of heart failure caused by acute myocardial infarction. Methods A total of 90 patients with myocardial infarction and heart failure were collected from 2009 to 2012 in this hospital,28 cases were level Ⅱ ,38 cases were level Ⅲ ,24 cases were level Ⅳ. 109 cases were as normal control group. The blood level of NT-proBNP in patients was tested,and the NT-proBNP in different levels （ Ⅱ,Ⅲ,Ⅳ） in heart failure were compared. Results NT proBNP in patients with acute myocardial infarction and Ⅱ level heart failure was（1 328±847）pg/mL,（4 887±1191）pg/mL for Ⅲ level,（26 605±5 636）pg/mL for Ⅳ level,each level was obviously higher than normal control group （P〈0.05）. Between different levels of heart failure the result was significantly different （P〈0.05 ）, the higher the level of heart failure was, the higher the level of NT-proP, NP was. After treatment, 38 cases with cardiac func-tion improved the NT proBNP level significantly reduced （P〈0.05）. Conclusion NT - proBNP in patients with acute myocardial infarction can be used as indicators of heart failure, and its level in evaluation of cardiac function and prognosis has important clinical value.
