孕妇生殖道B族链球菌感染与胎膜早破的关系及其对母儿预后的影响

摘　　要：目的探讨孕妇生殖道B族链球菌(GBS)感染与胎膜早破的关系及其对母儿预后的影响。方法收集2012年6～12月因胎膜早破在该院住院治疗的孕妇120例,同期住院的同孕周正常妊娠孕妇150例作为对照组,取阴道和肛拭子应用实时定量PCR进行GBS的检测,并将孕妇GBS阳性的胎膜早破患者与阴性患者所产的新生儿的预后作对比,了解孕妇生殖道GBS感染对母儿预后的影响。结果发生胎膜早破的患者GBS感染率(19.17%),未破膜的正常妊娠者GBS为5.33%,二者比较差异有统计学意义(P<0.05)。同时感染GBS阳性的孕妇比阴性孕妇其新生儿发生胎儿窘迫、新生儿肺炎、绒毛膜羊膜炎的发生率高且差异有统计学意义(P<0.05)。结论孕妇生殖道GBS感染为胎膜早破的高危因素,GBS的感染增加了胎儿窘迫、新生儿感染、产褥感染的发生率。Objective To investigate the relation between group B streptococcus infection and premature rupture membranes and influence in the health of pregnant and the newborns. Methods Collect from June 2012 to December 2012 for premature rupture of membrane in our hospital treatment of 120 cases of pregnant women,while in the hospital with pregnant women telomerase preg-nancy 150 cases as control group, take vaginal swab and anal swab application real-time quantitative PCR for GBS detection, and the woman GBS positive premature rupture of membrane and negative patients with the produce of neonatal prognosis for contrast, un-derstand pregnant women genital tract GBS infection on the influence of pregnant and the newborns. Results premature rupture of membrane in patients with GBS rates（19. 17 %）, no infection GBS pregnancies （5.33%）, compared with the statistical difference （P〈0.05）. At the same time,the pregnant women infected with GBS positive than negative pregnant women the neonatal happen fetal distress and neonatal pneumonia,chorioamnionitis incidence is high,and statistically significant（P〈0.05）. Conclusion GBS genital tract infection pregnant women for premature rupture of membrane high-risk factors, GBS infection increased fetal distress and neonatal infections,puerperal infection incidence.
