摘 要:目的对娱乐场所女性服务人员进行艾滋病健康干预,评估干预效果。方法选择县城范围内的32家发廊、卡拉OK厅、桑拿洗浴中心、足浴/足疗中心及宾馆/酒店的199名女性服务人员作为干预组,相邻乡镇的28家娱乐场所的157名女性服务人员作为对照组。根据自愿、知情同意原则,对199名干预组对象开展为期6个月的艾滋病干预(包括手机短信、同伴教育等干预措施)并评价其干预效果。结果与干预前相比,干预组在干预工作结束后0.5年内艾滋病知识知晓率(43.13%,P<0.01)、最近1次(78.13%,P<0.01)和最近1月(81.86%,P<0.01)与非固定对象(配偶)发生性行为时的安全套使用率、接受艾滋病咨询检测的比例(58.75%,P<0.01)、愿意到正规医疗机构就诊的比例(73.13%,P<0.01)及性病检出率(13.75%,P<0.05)均有明显改善;但在干预工作结束1.5年后,干预组仅艾滋病知识知晓率(31.76%)高于干预前(12.56%,P<0.01)及同期对照(20.83%,P<0.05),而最近1次(57.43%)和最近1月(56.08%)与非固定对象(配偶)发生性行为时的安全套使用率、求医行为及性病检出率(17.57%)均与同期对照组差异无统计学意义(P>0.05)。结论艾滋病干预近期效果显著但中远期效果欠佳,保持艾滋病干预工作的长期性和连续性对艾滋病的防制显得尤为重要。Objective To conduct HIV/AIDb health behavior interventions among iemales serviced m entertainment places, and to evaluate the effectiveness of the intervention. Methods One hundred and ninety - nine female sex workers serviced in 32 entertainment places of Lianyuan, including hair salons, Karaoke bars, sauna centers, foot bathing and foot massage centers, and hotels were served as intervention group, and 157 female sex workers serviced in entertainment places of the neighbor town were served as control group. According to the principle of voluntary informed consent, 6 - month interventions (mobile phone short messages and peer education) were enforced in 199 females of intervention group. And the effectiveness of the interventions was assessed. Results As compared with before the interventions, the awareness rate of HIV/AIDS knowledge (43.13%, P〈 0.01), the rate of condom use in the last sexual behavior (78. 13%, P〈 0.01) and in the last month (81.86 %, P 〈 0.01 ) when to engage in sex with non- regular partners, the proportion of receiving HIV counseling and testing (58.75%, P〈0.01), and behavior for seeing doctors in formal medical institutions (73.13%, P〈0.01), and the detection rate of venereal diseases (13.75 %, P〈 0.05) of intervention group were all significantly improved within six months after the interventions. But 18 months after the interventions, only the awareness rate of HIV/AIDS knowledge of intervention group (31.76%) was higher than those of the preintervention (12.56%, P〈0.05) and control group (20.83%, P〈0.05). No statistically significant difference was found in the rate of condom use in the last sexual behavior (57.43 % ) and in the last month (56.08 % ) when to engage in sex with non - regular partners, health seeking behavior and the detection rate of venereal diseases (17.57 % ) between intervention group and control group (P 〉 0.05 ).
