不同浓度七氟醚对精准腰椎间孔减压术诱发肌电图阈值的影响

摘　　要：目的比较不同浓度的七氟醚对精准腰椎间孔减压术(IO-Flex)诱发肌电图阈值的影。方法选择2012年4月～2012年10月择期行腰椎IO-Flex术患者60例,根据呼吸末七氟醚浓度随机分为3组,分别为1.5MAC七氟醚组、2.0MAC七氟醚组和2.5MAC七氟醚组,每组各20例。比较3组精准椎间孔减压术中脊神经根诱发肌电图阈值及脑电双频指数的变化。结果 2.5MAC七氟醚组脊神经根诱发肌电图阈值[(14.2±2.4)mA]高于1.5MAC七氟醚组[(7.3±1.5)mA]和2.0MAC七氟醚组[(7.7±1.8)mA],差异均有统计学意义(P<0.05),而1.5MAC七氟醚组和2.0MAC七氟醚组间差异无统计学意义(P>0.05);三组脑电双频指数差异无统计学意义(P>0.05)。结论 2.5MAC七氟醚组在IO-Flex术中可明显提高腰脊神经根的诱发肌电图的阈值。Objective To compare effects of different concentration of sevoflurane on threshold of evoked EMG in IO- Flex surgery. Methods 60 patients with IO-flex surgery from April 2012 to October 2012 were selected and randomly assigned to 1.SMAC group, 2.0MAC group and 2.SMAC group with 20 cases in each group. The changes in the nerve roots＂ threshold of evoked EMG and bispectral index were compared. Results The nerve roots＂ threshold of evoked EMG in 2.SMAC group [（14.2±2.4） mA] were higher than those in 1.SMAC group [（7.3±1.5） mA] and 2.0MAC group [（7.7±1.8） mA], the difference were statistically significant （P 〈 0.05）; the differences of nerve roots＂ threshold of evoked EMG in 1.SMAC group and 2.0MAC group were not statistically significant （P 〉 0.05）; the difference of bispectral index in 3 groups was not statistically significant （P 〉 0.05）. Conclusion 2.SMAC concentration of sevoflurane can increase the nerve roots＇ threshold significantly in IO-Flex surgery.
