双J输尿管支架管在上尿路梗阻中的应用研究

摘　　要：目的探讨逆行留置双J输尿管支架管在上尿路梗阻疾病的治疗效果。方法回顾性分析新疆医科大学第二附属医院2011年1月～2012年10月上尿路疾病致输尿管梗阻引起的肾积水患者52例，分析其成功逆行留置双J输尿管支架管治疗肾积水前后的治疗效果。结果治疗前、治疗后4周、治疗后6周B超提示肾盂集合系统分离分别为（26．9±5．5）、（22．7±5．5）、（22．2±7．5）mm。内引流效果良好，留置双J输尿管支架管后4、6周总体上较治疗前肾积水明显改善（P〈0．05）。各病种在留置支架管治疗前与治疗后4周、治疗前与治疗后6周差异有统计学意义（P〈0．05），在肿瘤组中治疗后4周与治疗后6周比较差异有统计学意义（P〈0．05）。结论留置双J输尿管支架管可明显改善肾积水。能缓解肾积水所致的腰部疼痛症状，值得临床推广。Objective To investigate, therapeutic effect of the antidromic indwelling double J ureteral stent in upper urinary tract obstruction disease. Methods A retrospective analysis of 51 patients with upper urinary tract disease caused by ureteral obstruction causing hydronephrosis in the Second Affiliated Hospital of Xinjiang medical university from 2011 January to 2012 October were analyzed, successful retrograde indwelling double J ureteral stent treatment of hydronephrosis before and after treatment were analyzed. Results Before treatment, after 4 weeks of treatment, after 6 weeks of treatment, ultrasound imaging prompted pelvis separation of the collection system were （26.9＋5.5）, （22.7＋5.5）, （22.2＋7.5） mm. Internal drainage effect is good, after 4 weeks and 6 weeks treatment hydronephrosis were improved than before on the whole （P 〈 0.05）. Compared with each other among before treatment, after 4 weeks of treatment, after 6 weeks of treatment, before treatment and after treatment of 4 weeks and 6 weeks were significant differences. Between after 4 weeks of treatment and after 6 weeks of treatment differences wwa not statistically significant, but the differences was statistical significance in the tumor group after 4 weeks of treatment and after 6 weeks of treatment. Conclusion Indwelling double J tube can obviously improve the hydronephrosis, remission of hydronephrosis caused by lumbar pain symptoms, worthy of clinical application.
