摘  要:目的:探讨基于三级医院-社区糖尿病管理模式的应用效果。方法:利用居民电子健康档案平台,建立南通市学田社区糖尿病患者电子信息数据库。选择数据库中糖尿病患者420例作为研究对象,随机分为观察组202例,对照组218例。观察组采取三级医院-社区糖尿病管理模式对患者进行教育管理;对照组采取单纯社区医师对患者教育管理。两组治疗3个月后,观察血糖、血脂和血压等相关指标改善情况。结果:(1)观察组体重指数22.1±1.6kg/m2,空腹血糖6.8±1.3mmoL/L,餐后2h血糖8.3±1.7mmoL/L,糖化血红蛋白(6.7±0.6)%,甘油三酯1.72±0.40mmoL/L。(2)对照组体重指数24.9±1.9kg/m2,空腹血糖8.3±1.6mmoL/L,餐后2h血糖9.7±2.0mmoL/L,糖化血红蛋白(7.5±0.8)%,甘油三酯2.26±0.27mmoL/L。两组以上5项指标比较差异均有统计学意义(P<0.05)。结论:观察组患者的代谢指标改善效果优于对照组,信息化条件下三级医院-社区糖尿病管理模式值得探索。Objective: To investigate the effects of tertiary hospital-community management style on diabetic pa- tients. Methods: Electronic information database of diabetic patients in Xuetian Community of Nantong City was estab- lished by the use of the electronic health records platform of the residents. 420 type 2 diabetes from the database were ran- domly divided into the tertiary hospital-community management group (Management group, 202 cases) and the community management group (Regular group, 218 cases). Metabolism associated index were tested before and after 3-month treat- ments. Results: Body mass index(BMI), triglycerides(TG), fasting plasm glucose (FPG), 2 hour postprandial glucose(2hPG), and glycosylated hemoglobin Alc(HbAle) in the Management group after treatments were all lower than those in the Regu- lar group (P〈0.05). Conclusions: Metabolism associated index in the Management group were improved better than in the Regular group, and the tertiary hospital-community diabetes management model is worth exploring under the conditions of informatization.
