摘  要:目的应用二维斑点追踪成像(STI)评价新生猪缺氧缺血性脑病(HIE)模型损伤心肌圆周运动,观察其病理变化。材料与方法 18只新生约克猪随机抽签分为对照组(n=8)和模型组(n=10),模型组行气管插管、低氧通气及双颈动脉夹闭,对照组假手术、不予低氧及夹闭。造模24h测量整体、基底、中间、心尖水平圆周应变及圆周应变率(SrC)峰值,包括圆周应变峰值(SC)、收缩期圆周应变率峰值(SrCS)、舒张早期圆周应变率峰值(SrCE)、舒张晚期圆周应变率峰值(SrCA),测量心率(HR)、左心室射血分数(LVEF)及Tei指数。对脑及心肌组织行病理检查。结果模型组新生猪造模后相继出现发绀、抽搐、颈强直等改变,对照组无明显行为异常,两组新生猪均无死亡。对照组脑神经元细胞完整、清晰,模型组脑神经元细胞变性、坏死,造模成功。与对照组比较,模型组各水平SC、SrCS、SrCE、SrCA均减低,HR加快,LVEF下降,Tei指数升高,差异均有统计学意义(P<0.05)。模型组心肌细胞发生变性、坏死及凋亡,对照组心肌细胞整齐、清晰。结论模型早期心肌圆周运动减低,心功能下降,心肌细胞出现变性、坏死及凋亡,心肌圆周应变及应变率检测可早期评价新生猪HIE合并心肌损伤。Purpose To evaluate the changes of circumferential motion using speckle tracking imaging (STI), and correlate with pathology of the damaged myocardium of newborn pigs with hypoxic-ischemic encephalopathy (HIE). Materials and Methods Eighteen newborn pigs were randomly divided into control group (n-8) and experimental group (n=10). The bilateral carotid arteries were blocked and 6% oxygen given mechanically in the experimental group. Sham operation was performed in the control group. After 24 hours, the peak value of circumferential strain (SC) and circumferential strain rate (SrC S, SrC E, SrC A) at each level were measured using STI, and the heart rate (HR), LVEF, Tel index were also measured. The pathological slices of the myocardium and brain were analyzed. Results Cyanosis, convulsions and neck stiffness were observed in experimental group; no abnormality was seen in the control group. Brain neurons cells were intact in control group while degeneration and necrosis were found in the experimental group to confirm successful modeling. Compared with the control group, SC, SrC S, SrC E, SrC A of each level and LVEF were decreased, while HR and Tel index were increased in the experimental group (P〈0.05). In the experimental group, degeneration, necrosis and apoptosis were found in some myocardial cells but intact and well defined in the control group. Conclusion Decreased circumferential motion and heart function in the early stage of HIE model is consistent with pathology. It is a reliable method to evaluate the newborn pigs' with HIE and myocardial damage, and it can provide experinaenlal study for researching HIE combined with myocardial damage in newborns.
