摘  要:目的应用定量组织速度成像技术评价甲状腺功能减退症患者的左心功能。资料与方法对30例甲状腺功能减退症患者及30例健康体检者(对照组)行三维超声检查,观察心尖两腔心切面、心尖四腔心切面和心尖左心室长轴图像,应用定量组织速度成像技术记录受试者二尖瓣环水平舒张及收缩功能指数。结果甲状腺功能减退症患者二尖瓣环处及6个室壁平均收缩期峰值速度(Vs)明显降低,部分室壁二尖瓣环处舒张早期峰值速度(Ve)、左心房收缩期峰值速度(Va)及Ve/Va明显降低,与对照组比较差异有统计学意义(P<0.05)。结论定量组织速度成像技术可以敏感、直观、定量评价甲状腺功能减退症患者的心脏舒张及收缩功能,早期发现无症状性心功能改变。Purpose To evaluate the left ventricular function in patients with hypothyroidism using quantitative tissue velocity. Materials and Methods Thirty patients with hypothyroidism and thirty normal controls were included in the study. Quantitative real-time three-plane tissue velocity imaging (apical four chamber, two-chamber and long axis) and quantitative tissue velocity imaging technique were performed to measure the diastole and systole indexes at the level of posterior mitral annulus. Each parameter was compared between the two groups. Results In hypothyroidism group, the average peak systolic velocity at mitral annulus was lower than that in control group (P〈0.05). In some walls the peak early diastolic velocity (Ve) and left atrial systolic velocity were significantly different between the two groups (P〈0.05). Conclusion Quantitative tissue velocity imaging is sensitive, intuitive, quantitative for evaluating the diastolic and systolic function in patients with hypothyroidism for early detection of asymptomatic changes.
