摘 要:目的比较空气与纯氧复苏窒息新生儿的临床效果。方法 70例窒息足月新生儿随机均分为空气复苏组(A组,先行空气复苏,如失败则改为纯氧复苏)和纯氧复苏组(B组,直接行纯氧复苏)。观察并记录出生1-、5-和10-min的Apgar评分、出生时脐动脉血气分析、第1次啼哭的时间、建立自主呼吸的时间、出生5min时的心率、达到SpO2>90%的时间、复苏失败率及1周内窒息新生儿死亡率。结果 A组第1次啼哭的时间和建立自主呼吸的时间较B组短[(1.43±0.62)minvs.(1.92±0.71)min和(4.93±1.46)min vs.(6.12±1.55)min](P<0.05);两组出生1-、5-和10-min的Apgar评分、出生时脐动脉血气分析指标、5min时心率、达到SpO2>90%的时间和复苏失败率均无统计学差异(P>0.05)。两组复苏的新生儿1周内均存活。结论空气复苏可用于足月窒息新生儿,近期效果与纯氧复苏相似。Objective To investigate the clinical efficacy of room-air versus oxygen administration in the resuscitation of depressed newborns. Methods Seventy full-term newborns with birth asphyxia were chosen and equally randomized into two groups of A(given room-air resuscitation firstly, followed by 100% oxygen resuscitation if failled) and B(given 100% oxygen resuscitation all the time). Apgar scores after birth, umbilical arterial blood gas analysis at birth, the time to start the first cry,the time to establish regular spontaneous breath, heart rate at five minutes after birth, the time to reach blood oxygen saturati0n(SpO2 ) above 90~, the mortality within 1 week after birth and the failure rate of resuscitation were recorded and compared. Results The times of the first cry and establishing regular spontaneous breath were less in group A than those in group B[(1.43±0. 62) min vs. (1.92±0. 71) min and (4. 93 ±1.46) min vs. (6. 12±1.55) mini (P〈0. 05). There were no significant differences in 1-min,5-min and 10-min Apgar scores, umbilical arterial blood gas at birth, heart rate at five minutes after birth, the time to reach SpO2 〉90 % and the failure rate of resuscitation between two groups(P〉0. 05). The newborns were all survival in a week. Conclusion Room-air resuscitation may be used in full-term newborns with neonatal asphyxia, which is as effective as 100 % oxygen in short-term efficacy of resuscitation.
