摘  要:目的评价阴囊内结构超声在男性不育症诊断中的临床应用价值。方法回顾分析118例临床诊断为男性不育症患者的阴囊声像图特点。结果 118例男性不育症患者,阴囊声像图表现正常15例(12.7%);精索静脉曲张41例(34.7%),声像图表现为精索蔓状静脉丛迂曲走形的管状结构;睾丸体积偏小13例(11.0%),声像图表现为睾丸各径线及体积明显减小;睾丸网扩张2例(1.7%),睾丸输出管扩张2例(1.7%),附睾管扩张19例(16.1%),声像图均表现为病变区的网格样无回声区;输精管增粗7例(5.9%),声像图表现为输精管均匀性或不均匀性增粗;先天性输精管缺如9例(7.6%),声像图表现为阴囊段输精管缺失;附睾囊肿39例(33.1%),声像图表现为附睾部圆形或椭圆形的无回声区;睾丸微石症11例(9.3%),声像图表现为睾丸实质内的点状强回声;睾丸鞘膜腔积液6例(5.1%),声像图表现为睾丸鞘膜腔内的无回声区;睾丸鞘膜腔结石3例(2.5%),声像图表现为睾丸鞘膜腔内游离点状或团块状高回声。结论阴囊超声是男性不育症的首选影像学检查,合理应用阴囊超声对男性不育症的诊断和治疗具有重要意义。Objective To evaluate the clinical application value of scrotal ultrasonography in the diagnosis of male sterility.Methods The scrotal ultrasonographic features of 118 cases of male sterility were retrospectively analyzed.Results Among 118 cases, 15 cases were negative( 12.7% ). Varicoeele was detected in 41 cases( 34.7% ), showecl tortuous ducts in spermatie cord andregurgitation in sonography. Testicular size reduced in 13 cases (11.0%). Two cases of rete testis dilation (1.7%), 2 cases of ductuli efferentes testis dialation( 1.7% ), 19 cases of epididymis dialation with netlike echo in lesion area were detected( 16.1% ).Seven cases of spermduct became thickened (5.9%),9 cases of congenital absence of vas deferentia (7.6%),39 cases of epididymal cysts with round or oval anechoic area in epididymis (33.1%). Eleven cases testicular microlithiasis showing multiplehyperechoic spots in testis (9.3%),6 cases of hydrocele testis (5.1%) and 3 cases of stone in tunica vaginalis cavity (2.5%). Conclusion Scrotal ultrasonography can be used as the routine imaging madality for male sterility, and it is of great importancefor the early diagnosis and therapy of male sterility.
