声诺维使用中的不良反应分析及安全管理探讨

摘　　要：目的探讨声诺维在超声造影使用中的不良反应及防治措施。方法回顾我院行超声造影检查的462例患者,分析造影过程中发生的不良反应,并制定相应的防范措施及预案。结果本组不良反应发生率为3.46%(16/462)。发生重度不良反应1例,出现过敏性休克早期表现:头痛、心悸、出汗、面色苍白、脉速而弱,后发展为发绀,脉搏消失,血压降至0,伴肢冷寒颤,经积极抢救好转;中度不良反应4例,表现为皮疹、瘙痒1例,胸闷不适2例,注射部位青肿1例,经对症处理恢复;轻度不良反应11例,表现为头痛1例,头晕2例,非特异性全身不适2例,注射部位疼痛6例,休息后自行恢复。结论应用声诺维行超声造影相对安全,严重不良反应少见,但超声造影过程中应严格掌握禁忌症和注意事项,配有相应的监护和抢救设备。Objective To discuss the adverse reactions and preventiond methods associted with SonoVue when contrast-enhanced ultrasound （CEUS） was used. Methods Data of 462 patients who underwent CEUS with SonoVue were retrospectively analyzed. Results The total incidence rate of adverse reactions were 3.46% （16/462）. Serious adverse reaction such as early anaphylactic shock was occurred in only one patient. Moderate adverse reaction occurred in 4 patients, symptoms including 1 case of erythra and pruritus,2 cases of dyspnea, 1 case of injection site bruising. They all were recovery by corresponding treat. Mild adverse reactions occurred in 11 cases,including 1 case of headaclle, 2 cases of dizziness, 2 cases of non-specifical unwell, 6 cases of injection site bruising. They all were recovery by taking a rest. Conclusion SonoVue is relatively safe, serious adverse reactions is rare. Contraindications should be controlled strictly. Monitoring and rescue equipments are necessary when CEUS was applicated.
