摘  要:目的探讨能量多普勒三维血管成像技术(3D-PDA)在脐带附着异常诊断中的应用价值。方法对50例常规超声检查疑为脐带附着异常的胎儿行3D-PDA检查并随访,将3D-PDA结果与产后胎盘情况进行比较。结果 3D-PDA诊断脐带附着异常46例,其中球拍状胎盘(脐带边缘附着)38例,3D-PDA显示脐带边缘附着,多角度可见血管树从附着点逐渐深入胎盘;帆状胎盘(脐带帆状附着)8例(3例合并有副胎盘),3D-PDA显示脐带帆状附着,多角度可见脐带根部附着于胎膜上,附着处未见胎盘组织。3D-PDA诊断结果与产后结果一致,诊断准确率100%。结论 3D-PDA能准确、直观地显示脐带附着点及其走行情况,在脐带附着异常的诊断中有重要价值。Objective To explore the value of three-dimensional power Doppler angiography (3D-PDA) in the diagnosis of abnormal attachment of umbilical cord. Methods Fifty suspected cases of abnormal umbilical cord attachment diagnosed by conventional ultrasound were included in this study. 3D-PDA was performed for every fetus. The 3D-PDA results were compared with the pathology of postpartum placental and umbilical cord. Results Forty-six cases of 50 abnormal placenta were confirmed by postpartum pathology, including 38 cases of racket-shaped placenta (umbilical cord edge attachment), and 8 cases (3 cases with vice placenta) of velamentous placenta (umbilical cord velamentous attachment). All 46 cases of umbilical cord of abnormal attachment were diagnosed correctly by 3D-PDA,the diagnostic accuracy rate was 100%. Conclusion 3D- PDA plays an important role in the diagnosis of abnormal attachments of umbilical cord.
