摘　　要：目的探讨获得性免疫缺陷综合征(AIDS)合并恶性肿瘤的超声表现及其诊断价值。方法回顾性分析我院5年间收治的14例经超声引导下病理组织学检查证实为AIDS合并恶性肿瘤患者的超声资料,观察淋巴结大小、纵横比、边界、形态及血流。结果 14例AIDS患者中,合并非霍奇金淋巴瘤9例,卡波西肉瘤2例,肝细胞肝癌、结肠腺癌及宫颈癌各1例。淋巴瘤以颈部、颌下及腋下淋巴结多见,其次为肝脏、脾脏、腹股沟、腹腔、肾上腺及乳腺,超声显示病灶为分布均匀的中低回声,圆形或椭圆形病灶,血流丰富。腹腔淋巴结卡波西肉瘤1例,颈部及腹股沟淋巴结卡波西肉瘤1例,受累淋巴结回声呈较均匀的稍高回声。12例患者经超声引导下穿刺活体病理组织学检查,均无严重并发症发生。结论常规超声检查对诊断AIDS合并恶性肿瘤有重要价值,超声引导下穿刺活检定位准确、安全,诊断准确率高,超声可作为首选的影像学检查方法。Objective To explore the dagnositie value and image characteristics of malignant tumors in patients with acquried immunodefieieney syndrome （AIDS）. Methods Fourteen AIDS patients with malignant tumors confirmed by ultrasound guided biopsy in last five years in our hospital were included in this study. Imaging datas related to lymph nodes were retrospectively analysed. Results In 14 patients, 9 cases of non-Hodgkin lymphoma, 2 cases of Kaposi Sarcoma, 1 case of liver canee, 1 case of colon cancer, 1 case of cervical carcinoma were detected by ultrasound imaging. Lymphoma mainly located in neck, armpit, then liver, spleen and groin area, and then peritoneal cavity or adrenal glands.Round or oval shape of lesions with homogeneous equal-echo or hypoecho are main characteristics of tumour lesions. Abundant color signals were detected in most lesions. 1 ease of Kaposi Sarcoma located in peritoneal cavity lymph nodes, and the other case located in neck and groin area lymph nodes. 12 patients were underwent ultrasound guided biopsy, no complication was found. Conclusion Ultrasound image characteristics and ultrasound guided biopsy are helpful to diagnose malignant tumors in patients with AIDS.
