摘  要:目的观察剖宫产术中使用缩宫素对产妇血流动力学的影响。方法腰麻下行急诊剖宫产产妇40例,待胎儿娩出脐带夹毕后,静脉注射缩宫素10U,20s注完。采用桡动脉有创测压及阻抗心动图监测并记录缩宫素注射前(基础值)和注射后3min内每15秒的MAP、HR、加速度指数(ACI)、每搏输出量(SV)、左心作功量(LCW),计算体循环阻力(SVR)。结果与基础值比较,缩宫素注射后MAP、SVR明显下降,HR明显增快,ACI、LCW明显增加(P<0.05),SV变化差异无统计学意义。结论缩宫素可引起产妇MAP、SVR下降及HR增快、ACI、LCW增加。Objective To observe the effects of oxytocin on hemodynamies in paturients undergoing cesarean section. Methods Forty parturients undergoing emergency cesarean section with spinal anesthesia received 10 units (U) oxytocin within 20 s after umbilical occlusion. Invasive radial arterial pressure and impedance cardiography (ICG) were used to investigate MAP, HR, SV, acceleration index (ACI), and left cardiac work (LCW) before oxytocin administration (baseline) and every 15 seconds following oxytocin administration. SVR was also calculated. Results Compared with baseline, MAP and SVR decreased whereas HR, ACI, and LCW increased after oxytocin administration (P 〈0.05). SV had no statistical significance after administration. Conelmion Oxytocin may result in decreasing in MAP and SVR, and increasing in HR, ACI and LCW.
