摘  要:目的调查分析铜陵市2011年8月发生的一起因游泳引起学生咽结膜热暴发的病原学病因和流行因素,为今后此类传染病的防控工作提供借鉴。方法制定病例定义进行病例搜索,对所有病例进行流行病学调查,对游泳馆进行现场卫生学调查,描述流行特征,分析流行因素;对患者咽拭子标本进行进行流感病毒核酸检测与呼吸道腺病毒特异性DNA核酸片段检测,以查找病原体。结果共发现咽结膜热病例31例,均为建安小学室内游泳馆暑假短期游泳培训班学员,罹患率为18.56%。发病主要集中8月4~9日,病例均为小学学生。临床特征为发热伴咽痛、结膜炎等。流行病学、现场卫生学调查与分析提示发病危险因素与游泳馆游泳池水污染有关。实验室检测结果为5例患儿中有4例咽拭子呼吸道腺病毒特异性DNA核酸片段检测阳性,证实为腺病毒感染。结论本起咽结膜热暴发疫情的病原体为腺病毒,暴发可能是通过被污染的游泳池水造成传播的。OBJECTIVE The present study was conducted to investigate the pathogenic and epidemic factors of an outbreak of pharyngoconjunctival fever and to provide basis for developing interventions.METHODS The established case definition and epidemiological questionnaires were used to investigate all the cases,the swimming pool was surveyed by hygienic evaluation.These data were used to describe epidemiological features and analyze epidemic factors of the cases.Influenza virus nucleic acid and respiratory adenovirus specific DNA detection were performed on patients’ swabs.RESULTS There were 31 cases of pharyngoconjunctival fever with a peak from August 4th to 9th,involving mostly primary school students,attack rate was 18.56%.The clinical characteristics were fever accompanied by angina and conjunctivitis.The epidemiological and hygienic investigation showed that the risk factors were associated with the pollution of swimming water.Adenovirus infection was revealed by pathogenic detection.CONCLUSION The pathogenic agent of pharyngoconjunctival fever breaking out is confirmed to be adenovirus,and the polluted water in swimming pool might be the main route of transmission.
