公立与私营医院常见单病种住院服务的比较

摘　　要：目的比较公立、私营医疗机构的收费水平、服务质量和效率。方法在公立与私营医院规模接近的情况下,选择单纯性阑尾炎、子宫肌瘤、卵巢囊肿等单病种作为研究病种。结果平均住院费用:鲁中单纯性阑尾炎公、私立医院无差异,鲁西公、私立医院分别为1399元和1065元,私营医院较低(P=0.012)。子宫肌瘤的平均住院费用鲁中地区个别私营医院略低,总体看公、私立医院无明显差异;鲁西公、私立医院分别为3844元和3150元,均数比较差异有统计学意义(P=0.013),私营医院较低。住院日:单纯性阑尾炎鲁西公、私立医院2006年分别为7.4d和4.1d,私营医院较低(P=0.002),2007年分别为4.9d和5.0d,无差别;子宫肌瘤,鲁中公、私立医院平均住院日分别为10.6d和8.0d,均数比较有差异,私立医院较短(P=0.002);鲁西公、私立医院比较无差异。卵巢囊肿的住院费用及住院日公、私立医院无差别。结论无论是住院费用还是住院日,私营医院较低。营利性医疗机构的存在,促使公立医院降低医疗服务费用,结果趋于二者无差别。公立医院住院日较长,与公立医院、特别是基层医院床位利用率不高有关,长住院日影响了长期的经济效益,公立医院管理应注重经济效率和社会效益相结合。OBJECTIVE To compare the charge level,service quality and efficiency between public and private medical institutions.METHODS The paper selected single diseases like simple appendicitis,hysteromyoma and ovarian cyst as its research subjects from public and private medical institutions of similar scales.RESULTS The average hospitalization expense：the cost of simple appendicitis had no difference between public and private hospitals in central areas of Shandong Province,while in western areas of Shandong Province,the cost of simple appendicitis in public and private hospitals was 1 399 Yuan and 1 065 Yuan individually：the private hospital cost was lower（P = 0.012）.The average hospitalization expense of hysteromyoma was lower in some private hospitals in the central areas,but generally speaking,there was no obvious difference between public and private medical institutions.In western areas of Shandong Province,the average hospitalization expense of hysteromyoma in public hospitals was 3 844 Yuan while in private hospitals 3 150 Yuan,and the mean comparison had a difference（P = 0.013）：the cost was lower in private hospitals.The average length of stay：in western areas of Shandong Province in 2006,stay time due to simple appendicitis was 7.4 days in public hospitals while 4.1 days in private hospitals：the stay time was shorter in private hospitals（P = 0.002）;in 2007,hospitalization stay due to simple appendicitis was 4.9 days in public hospitals while 5.0 days in private hospitals：there was no difference.In central areas of Shandong Province,the average hospitalization stay of hysteromyoma was 10.6 days and 8.0 days in public and private hospitals respectively,and the mean comparison had difference：the private hospital stay was shorter（P = 0.002）;in western areas of Shandong Province,there was no difference between public and private hospitals as to the average hospitalization stay of hysteromyoma.The hospitalization cost of oophoritic cyst had no difference between public and private hospi
