经内镜乳头括约肌切开术(EST)与开腹胆总管切开取石术治疗胆总管结石的疗效对比研究

摘　　要：目的比较经内镜乳头括约肌切开术(EST)与开腹胆总管切开取石术治疗胆总管结石的疗效。方法选择我院2009年9月～2012年9月收治的胆总管结石患者64例,根据手术方法不同分为治疗组(经内镜乳头括约肌切开术(EST))与对照组(开腹胆总管切开取石术)各32例,比较两组的平均手术时间、引流时间、术后患者下床活动时间、术后住院时间、并发症等。结果与对照组比较,治疗组患者手术时间短、术后下床活动时间早、引流时间短、术后住院时间短,差异有统计学意义(P<0.05);治疗组并发症的发生率(7.5%)明显低于对照组(P<0.05)。结论经内镜乳头括约肌切开术(EST)治疗胆总管结石具有创伤小、痛苦少、患者术后恢复快的优点,是治疗胆总管结石的理想手术方式之一,值得推广和应用。Objective To compare incision lithotomy efficacy of the treatment of common bile duct stones after endo- scopic sphincterotomy （EST） and open common bile duct. Methods A total of 64 cases in our hospital from September 2009 to September 2012 choledocholithiasis patients admitted into a treatment group according to different surgical methods （endoscopic sphincterotomy （EST）） and control group （open The abdominal the choledocholithotomy surgery） and 32 cases each, observed and compared between the two groups of average operative time, drainage time, postoperative ambulation, postoperative hospital stay, complications. Results Compared with the control group, the patients treated with surgery a short time earlier postoperative ambulation, drainage time, shorter postoperative hospital stay, with significant difference（P 〈 0.05）, the incidence of complications in the treatment group （7.5%） was significantly lower than that in control group （P 〈 0.05）. Conclusion The treatment of common bile duct stones after endoscopic sphincterotomy （EST） has a less invasive, less painful, patients and quick recovery, and is one of the ideal surgical approach for the treatment of common bile duct stones, worthy of promotion and application.
