右美托咪定在ICU重型颅脑外伤患者镇静中的应用

摘　　要：目的探讨右美托咪定在ICU重型颅脑外伤患者镇静中的应用及疗效分析。方法选取我院2009年11月～2011年2月接诊治疗的34例重型颅脑外伤躁动患者为观察组,给予右美托咪定镇静治疗;选取同期34例重型颅脑外伤躁动患者组成对照组,给予安定镇静治疗,治疗后对比两组Ramsay镇静评分,记录并分析观察组用药前及用药后10 min、30 min、1 h、6 h时心率、呼吸频率、动脉血氧饱和度及平均血压等生命体征变化。结果观察组34例患者接受右美托咪定镇静治疗1 h内Ramsay评分均达2～4分,较对照组镇静效果满意,且每隔1 h均能被唤醒,无镇静过度现象;用药后10 min、30 min、1 h、6 h的生命体征变化明显,心率、呼吸频率及平均血压明显下降(P<0.05),动脉血氧饱和度明显升高(P<0.05)。结论右美托咪定用于ICU重型颅脑外伤患者镇静治疗中可控性好、安全性高,是一种比较理想的镇静药物。Objective To evaluate the application and efficacy of dexmedetomidine on severe head trauma patients in the ICU sedation. Methods All of 34 patients with severe traumatic brain injury treatment admited from November 2009 to February 2011 in our hospital restless patients were chosen as the observation group, given dexmedetomidine treatment in our hospital. Selected the same period 34 patients with severe head trauma restlessness patients composed the control group given diazepam, compared Ramsay scores between the two groups after treatment. After treatment, the Ramsay sedation score of 2-4 was divided into the satisfaction of the sedative effect, recorded and analyzed before treatment and after 10 min, 30 min, lh, 6 h heart rate, respiratory rate, arterial oxygen saturation and the average blood pressure and other vital signs. Results All 34 patients received dexmedetomidine given sedation 1 h Ramsay score reached 2-4, the effect of satisfaction compared to the control group, and every 1 h can wake without excessive sedation phenomenon; medication after 10 rain, 30 min, 1 h 6 h, changes in vital signs, heart rate, respiratory rate and mean blood pressure decreased significantly （P 〈0.05）, and arterial oxygen saturation was significantly higher （P 〈 0.05）. Conclusion Dexmedetomidine given for ICU with severe head trauma patients sedation therapy is a controllable and safe method. Dexmedetomidine is an ideal sedative drugs.
