孕妇围产营养门诊创建与实施对妊娠期糖尿病患者妊娠结局的影响

摘　　要：目的探讨孕妇围产营养门诊创建与实施对妊娠期糖尿病患者妊娠结局的影响。方法应用随机数字表法将2011年7月1日～2012年7月31日在我院治疗的232例妊娠期糖尿病患者分为A组和B组,A组孕产妇给予常规治疗和护理,而B组孕产妇则同时接受由助产士坐诊的围产营养专科门诊营养饮食指导,比较两组患者妊娠结局。结果 B组孕产妇妊娠期高血压疾病、孕产妇糖尿病急性并发症、妊娠羊水过多、孕产妇孕期感染、巨大胎儿、新生儿呼吸窘迫综合征和新生儿低血糖发生率明显低于A组,有显著性差异(P<0.05),新生儿转新生儿监护病房治疗率明显低于A组,有显著性差异(P<0.05),两组围生儿在围生儿死亡和新生儿窒息发生率方面,无显著性差异(P>0.05)。结论孕妇围产营养门诊助产士进行饮食指导能够有效控制好妊娠期糖尿病患者的血糖,改善患者和围生儿的结局,值得推广。Objective To investigate the influence of the pregnancy outcome by creating and implementing perinatal nutrition outpatient service. Methods The clinical data of 232 gestational diabetes patients who were diagnosed and treated in our hospital were collected, then they were divided into group A and group B by random number table, pa- tients in group A were given routine treatment and nursing,patients in group B were given nutrition clinic diet guid- ance from maternity assistant in perinatal nutrition outpatient service based on routine treatment and nursing. The pregnancy outcome of gestational diabetes patients were evaluated. Results The incidence of gestational hypertension disease,the acute complications of diabetes,the polyhydramnios,the infection during pregnancy,the maerosomia,the neonatal respiratory distress syndrome and the neonatal low blood in group B were lower than that in group A （P 〈 0.05） ,but the incidence of NICU therapy in group B was lower than that in group A （P 〈 0.05）,the incidence of circumference begat death and neonatal suffocation were no different（P 〉 0.05）. Conclusion The nutrition clinic diet guidance from maternity assistant in perinatal nutrition outpatient service based on routine treatment and nursing can obviously effectively control blood sugar of gestational diabetes patients, and the patients and circumference begat end, it is worthy of applying for clinical.
