两种方法治疗剖宫产术中难治性产后出血的效果比较

摘　　要：目的比较子宫动脉结扎联合改良式B-Lynch缝合术、子宫动脉结扎联合宫腔纱布填塞术治疗剖宫产术中难治性产后出血的效果。方法回顾性分析我院2010～2012年间88例剖宫产术中难治性产后出血患者的临床资料。结果子宫动脉结扎联合改良式B-Lynch缝合术组止血有效率高于子宫动脉结扎联合宫腔纱布填塞术组,差异无统计学意义(P>0.05);子宫动脉结扎联合改良式B-Lynch缝合术组术后2 h阴道流血量、术后24 h阴道流血量均显著低于子宫动脉结扎联合宫腔纱布填塞术组,差异有统计学意义(P<0.05)。结论对剖宫产术中难治性产后出血患者,在行子宫动脉结扎的基础上进行改良式B-Lynch缝合术是一种非常有价值的治疗方法。Objective Compared effect between the uterine artery ligation combined with modified B-Lynch suture and the uterine artery ligation combined with intrauterine gauze packing which treated intractable postpartum hemor- rhage in cesarean section. Methods Eighty-eight cases of intractable postpartum hemorrhage patients in cesarean sec- tion from 2010 to 2012 in our hospital were retrospectively analyzed. Results The hemostatic effective rate of uterine artery ligation combined with modified B-Lynch suture group was higher than that of uterine artery ligation combined with intrauterine gauze tamponade group with no significant difference （P 〉 0.05） ; the amount of vaginal bleeding after 2 hours and 24 hours in uterine artery ligation combined with modified B-Lynch suture group were significantly lower than those of uterine artery ligation combined with intrauterine gauze tamponade group, the difference was statistically significance （P 〈 0.05）. Conclusion The uterine artery ligation combined with modified B-Lynch suture is a very valuable treatment method for the patients of intractable postpartum hemorrhage in cesarean sections.
