600例外剥内扎法加括约肌切断术治疗环形混合痔的临床治疗体会

摘　　要：目的：探讨外剥内扎法加括约肌切断术治疗环状混合痔的治疗效果和治疗体会。方法：将我院2010年2月至2011年8月收治的600例环形混合痔的患者分为两组，一组为观察组300例，应用外剥内扎加括约肌切断术治疗环状混合痔，另一组为对照组应用单纯外剥内扎术治疗，观察两组的治疗效果。结果：对所有患者术后随访1—2年，两组患者术后排便均通畅，无感染及大出血等并发症产生，但观察组术后创缘水肿、残留皮赘和肛门狭窄发生例数均低于对照组，两组差异均有显著性统计学意义（P〈0．005）。结论：外剥内扎法加括约肌切断术治疗环形混合痔是一种安全，有效地且并发症少的手术方法，值得临床推广应用。Objective：To investigate the stripping of Neizhafajia sphincter treatment effect and treatment experience in the treatment of circumferential mixed hemorrhoids cutting. Methods：the patients in our hospital from 2010 February to 2011 August were treated 600 cases of annular mixed hemorrhoids were divided into two groups, one group was 300 cases in observation group, application of external strip and internal ligation and sphincterotomy in treatment of circumferential mixed hemorrhoids, another group was control group by simple Milligan-Morgan operation, observe the therapeutic effects of two groups. Results： all the patients were followed up for 1 to 2 years, two groups of patients with postoperative defecation were patent, no infection and bleeding and other complications, the patients in the observation group, but after a margin of edema, residual skin tag and anal stenosis cases were lower than those in the control group, the two groups had significant difference statistically significant （P〈0.005）. Conclusion：external stripping Neizhafajia sphincterotomy in treatment of circumferential mixed hemorrhoids is a safe, effective operation method and less complica- tions, is worthy of clinical application.
