彩色多普勒超声对冠状动脉窦瘤破裂的诊断价值

摘　　要：目的评价彩色多普勒超声在冠状动脉窦瘤破裂中的诊断价值。方法分析24例冠状动脉窦瘤患者的超声心动图资料并与手术结果对照。结果 24例患者中,确诊23例(95.8%),误诊1例(4.2%)。确诊的23例患者中,起源于右冠状动脉窦17例(73.9%),其中12例破入右室,5例破入右房,起源于无冠状动脉窦6例(26.1%),全部破入右房。合并主动脉瓣中量以上反流12例,主动脉瓣脱垂3例,感染性心内膜炎2例,心包积液9例。结论彩色多普勒超声能对冠状动脉窦瘤的起源、部位、大小、血流动力学变化以及合并症的情况做出快速、准确的诊断,为临床选择治疗方法及预后随访提供重要依据。aneurysm echocardi Objective To evaluate the diagnostic significance of color doppler ultrasound in ruptured sinus of valsalva Methods Twenty-four patients with ruptured sinus of valsalva aneurysm were analyzed the feature of ography. Results Twenty-three cases were correctly diagnosed and 1 case was misdiagnosed in the 24 cases. There were 17 eases of right aortic sinus aneurysm, 12 cases among them rupture were penetrated into right ventricle, 5 cases were penetrated into fight atrium; there were 6 cases of null aneurysm of aortic sinus and all patients among them rupture was penetrated into right atrium. There were 12 cases with aortic insufficiency, 3 cases with aortic pro- lapse, 2 cases with infective endocarditis, and 2 cases complicated with pericardial effusions. Conclusion Color doppler ultrasound can quickly and precisely define the anatomy of the sinus of valsava aneurysms, including its ori- gin, location, size, hemodynamic changes and complications, which provide the basis for selecting treatment plan and estimating prognosis.
