应用自制穿刺针在腹腔镜小儿腹股沟疝修补术中的应用体会

摘　　要：目的介绍自制穿刺针在腹腔镜小儿腹股沟疝修补术中的临床应用体会。方法回顾性分析2011年1月至2012年1月，采用自制穿刺针结合专用器械行腹腔镜小儿腹股沟疝修补36例的病例资料。结果36例患者均顺利完成手术，单侧手术时间7～16min；术中发现对侧隐性疝7例，占19．4％，均同时修补。术后不输液，术前后不使用抗菌药物，无并发症发生，术后第2天出院；随访2～15个月，无复发。结论使用自制穿刺针结合专用器械行腹腔镜小儿腹股沟疝修补术，穿刺方便快捷、创口小、损伤小、安全、修补确切可靠、可发现对侧隐性疝并同时修补，有较好的临床应用价值。Objective To introduce a homemade puncture needle for laparoscopic pediatric inguinal hernia repair. Methods From January 2011 to January 2012, 36 children with inguinal hernia underwent laparoscopic inguinal hernia repair using homemade puncture needle. Their clinical data were respectively analyzed. Results The inguinal hernia repair was conducted successfully in all patients, the unilateral operation time ranged from 7 to 16 min. 7 patients （19. 4%） were revealed to have contralateral recessive hernia and underwent repair meanwhile. Not postoperative transfusion and use of antibiotics before and after surgery, no complications occurred in all and discharged at day 2 in post-operation. The follow-up time ranged 2 to 15 months and no recurrence was noted in them. Conclusion The homemade puncture needle combined with laparoscopic instruments for pediatric inguinal hernia repair; have some benefits including convenient performance, smaller size of wound, less injury to tissue, less pain, and quick recovery, as well as security. They also contribute to contralateral hernia repair with better clinical value.
