冠心病血瘀证的遗传流行病学研究

摘　　要：目的探讨冠心病血瘀证的遗传特征。方法运用病例对照的遗传流行病学家系调查方法,对冠心病血瘀证家系(观察组)1565例和冠心病非血瘀证家系(对照组)1553例进行分离比、遗传度、遗传模式及多基因遗传方式等遗传参数的计算分析。结果观察组亲属冠心病血瘀证的患病率(5.24%)显著高于对照组(1.29%),差异有统计学意义(P<0.05)。观察组一级亲属分离比为0.1296,符合多基因遗传病的特点。观察组一级亲属遗传度为58.04%,二级亲属遗传度为54.80%,属中度遗传。观察组一级亲属遗传度为0.5804时,其理论发病率为8.00%,与实际发病率5.89%呈接近趋势;二级亲属的遗传度为0.5480时,其理论发病率为6.20%,与实际发病率4.13%呈接近趋势。结论冠心病血瘀证具有一定的家族聚集的倾向,其在家系中的分布符合多基因遗传特征。Objective To explore the genetic characteristics of blood stasis pattern of coronary heart disease （CHD）. Meth- ods A case-controlled genealogical investigation of genetic epidemiology was used to calculate and analyze the genetic parame- ters such as separation ratio, heritability, genetic patterns and polygenic inheritance way of 1565 CHD patients with blood sta- sis pattern in the observation group and 1553 CHD patients without blood stasis pattern in the control group. Results The prevalence of CHD with blood stasis pattern of the relatives in the observation group was 5.24%, which was significantly high- er than 1. 29% in the control group （P〈0.05）. The segregation ratio of the first-degree relatives in the observation group was 0. 1296, which was in line with the characteristics of polygenic disease. The heritability of the first-degree and second-degree relatives in the observation group was 58.04 %and 54.80 % respectively, with a moderate hereditary. The theoretical incidence of the first-degree and second-degree relatives in the observation group was 8.00% and 6.20M respectively, which was close to 5. 890/6o and 4.13% of the actual incidence. Conclusion The blood stasis pattern of CHD has a certain tendency to familial ag- gregation and its distribution in pedigree meets the polygenic genetic characteristics.
