丹参注射液对急性心肌梗死家兔左心室重构和心功能的影响

摘　　要：目的研究丹参注射液对急性心肌梗死(AMI)模型家兔左心室重构及心功能的影响。方法采用结扎家兔左冠状动脉前降支建立AMI模型,造模成功32只家兔随机分为模型组、丹参注射液组各16只,另设假手术组16只,只穿线不结扎。丹参注射液组给予丹参注射液0.5ml/100g灌胃,每日1次,共3周,假手术组、模型组常规饲养。各组分别在术后1周、3周观察家兔一般情况、体表心电图(ECG)、左室收缩末压(LVESP)、左室舒张末压(LVEDP)、左室内压变化速率峰值(dp/dtmax)、心输出量(CO)和心室重量(HW)、心室重量指数(VWI)、左心室腔内直径(D)的变化。结果模型组家兔食量减少、活动少、反应能力差,ECG显示出宽大的病理性Q波,而丹参注射液组家兔食量恢复,病理性Q波仅有2只。术后3周模型组与假手术组比较,LVSEP、dp/dtmax和CO明显下降,LVEDP明显增加(P<0.05或P<0.01),丹参注射液组较模型组LVSEP、dp/dtmax和CO明显升高,LVEDP明显降低(P<0.05或P<0.01)。术后3周模型组与假手术组比较,HW、VWI升高,D明显增大(P<0.05或P<0.01);丹参注射液组与模型组比较,HW、VWI下降,D明显减小(P<0.05或P<0.01)。结论丹参注射液能改善AMI家兔左心室重构参数,增加心输出量,改善心功能。Objective To study the effects of Danshen Injection on the left ventricular remodeling （LVF） and cardiac func- tion （CF） in acute myocardial infarction （AMI） rabbits. Methods The AMI models were established by ligation of anterior descending branch of the left coronary artery in rabbits. Thirty-two successfully modeling rabbits were randomized into the model group and Danshen Injection group, with 16 in each. Another 16 rabbits without ligation were threaded only and chosen as the sham operation group. The Danshen Injection group was gavaged with 0.5 ml/100 g Danshen Injection once daily for three weeks. The sham operation group and model group were given the conventional breeding. The general condition, body- surface electrocardiogram （BS-ECG）, left ventrieular end-systolic pressure （LVESP）, left ventrieular end-diastolic pressure （LVEDP）, maximum rate of change in left ventricular pressure （dp/dtmax）, cardiac output （CO）, heart weight （HW）, ven- tricular weight index （VWI） and left-ventricular cavity diameter （D） were observed 1 week and 3 weeks after operation. Results The appetite, activities and response were decreased in model rabbits and the ECG showed large pathological Q waves. The food intakes of rabbits were recovered and only 2 pathological Q waves were left in the Danshen Injection group. Comparing with the sham operation group, the LVESP, dp/dtmax and CO was significantly decreased but the LVEDP was sig- nificantly increased in the model group 3 weeks after operation （P〈0.05 or P〈0, 01）. Comparing with the model group, the LVESP, dp/dtmax and CO was significantly increased but the LVEDP was significantly decreased in the Danshen Injection group （P〈0.05 or P〈0.01）. Comparing with the sham operation group, the HW, VWI and D was significantly increased in the model group 3 weeks after operation （P〈0.05 or P〈0.01）.
