高频超声对儿童髋关节滑膜炎的诊断价值

摘　　要：目的探讨高频超声检查在儿童髋关节滑膜炎临床诊断中的应用价值。方法选取本院2010年2月～2012年7月收治的30例髋关节滑膜炎患儿作为研究对象,采用GE500型超声诊断仪对患儿双侧髋关节病灶处进行检查,探讨高频超声检查在小儿髋关节滑膜炎临床诊断中的应用价值。结果本组30例患儿均行高频超声检查,结果患侧髋关节颈前间隙均较正常增宽,诊断阳性率为100%,无误诊病例。超声主要表现为患儿患侧或双侧髋关节处出现无回声关节积液,关节囊扩张外凸,关节间隙增宽,股骨颈强回声带与关节囊回声间距大于6mm,与健侧或正常对比差值大于2mm。结论高频超声具有无创、操作简便、经济等优点,十分适合小儿髋关节病变的诊断,且对髋关节滑膜炎具有较高的确诊率,可作为临床诊断小儿髋关节滑膜炎的首选检查手段使用。Objective To probe into the application value of high-frequency ultrasound in the clinical diagnosis of children with transient synovitis of hip. Methods Thirty eases of children with transient synovitis of hip admitted into our hospital between February 2010 to July 2012 were selected as participants. Their lesions located in bilateral hip joints were examined by ultrasonic diagnostic apparatus （GE 500） in order to explore the application value of high-frequency ultrasound in the diagnosis of children with hip synovitis. Results The group of 30 participants were all examined by high-frequency ultrasound. Outcomes displayed that the space in front of the hip joint neck in affected side was wider than that of normal one with 100% diagnostic positive rate and without missed ease. The images by ultrasound in the hip joint of the affected or bilateral sides were presented with anechoic joint effusion, outward expanding of joint capsule, widening of the joint space, the separation distance between the strong echo of the femoral neck and echo from joint capsule being over 6 mm and the difference between them was 2 mm longer compared with the unin- jured or normal one. Conclusion At the advantages of non-invasion, simple operation, and affordability, the high-frequency ultrasound is very suitable for the diagnosis of pediatric hip lesions and has a high rate in definite diagnosis of hip synovitis, which can be chosen as the preferred inspection method in the diagnosis of children with transient synovitis of hip in clinic.
