23个病种实施临床路径效果分析

摘　　要：目的了解临床路径管理对医院工作效率、经济效率及患者满意度的影响。方法选择2010年8月至2012年8月于我院住院的适合实施临床路径管理的患者，包括轻一中度支气管哮喘等23个病种的1436例作为观察组；选择2008年1月至2009年12月我院未曾实施临床路径的上述同样疾病的1445例患者作为对照组，以住院总费用、药物总费用、平均住院时间、药品种数、术后生活回归时间及满意度评分等作为评价指标。结果观察组的住院总费用、药物总费用、抗生素费用、平均住院时间、抗生素使用时间、药品种数、术前住院时间、术后住院时间及术后生活回归时间等指标均低于对照组（P〈0．05），满意度评分则高于对照组（P〈0．05）。结论临床路径能科学、有效、合理地控制医疗成本，提高医院运行效率，提高患者满意度。Objective To study the effects of clinical pathway management on hospital efficiency, cost effectiveness and patient satisfaction. Methods Choosing inpatients appropriate for clinical pathway management who were admitted from August 2010 to August 2012 into the hospital, including those patients with any of the 23 diseases such as bronchial asthma（mild or moderate）at the hospital, totaling 1436 cases as the observation group. Choosing those inpatients who were admitted from January 2008 to December 2009 into the hospital and not subject to the clinical pathway management, including those patients diagnosed with any of the above 23 diseases, totaling 1445 cases as the control group. Total hospital expense,total medicine expense, average days of stay, types of drugs used, days of normal life recovery after operation, and satisfaction score were made the evaluation indexes. Results Such indexes as the total hospital expense, total medicine expense, antibody expense, average days of stay, types of drugs used,hospital days before operation, days of normal life recovery after operation were lower than the control group（P〈0. 05）, while the satisfaction score was higher than the control group（P〈0. 05）. Conclusion Clinical pathway can control medical costs in a scientific, effective and reasonable way, while improving hospital efficiency and raising patient satisfaction.
