P-选择素水平及血小板计数在重症感染患者的临床价值

摘　　要：目的探讨P-选择素水平、血小板计数在评价、预测重症感染患者病情变化及预后的，临床意义。方法选择2011年9月至2012年9月郑州大学第一附属医院呼吸ICU病房重症感染患者80例，按ACCM／SC—CMl991年芝加哥会议上提出的全身炎症反应综合征（SIRS）诊断标准，分为SIRS组40例（A组）和非SIRS组40例（B组）；选择同期健康体检者40例作为对照组（C组），比较各组间的P-选择素、血小板计数的差异；A组按预后分为生存组和死亡组，比较两组的P-选择素、血小板计数的差异。结果A组与B组、c组比较，P-选择素升高，血小板计数降低，差异有统计学意义；B组P选择素较c组升高，差异有统计学意义；死亡组与生存组比较，P一选择素升高，血小板计数降低，差异有统计学意义。结论P．选择素水平、血小板计数可以作为反映重症感染患者病情严重程度及预后的可靠指标。Objective To investigate the P-selectin and platelet count in the evaluation, predic- tion of severity and prognosis of patients with severe infection and its clinical significance. Methods Eighty patients in RICU of the first affiliated hospital of Zhengzhou university forsevere respiratory infections from September 2011 to September 2012 were divided into SIRS group（ group A） and non-SIRS group of 40 patients （ group B） according to the criteria for the diagnosis of system in- flammatory response syndrome （SIRS） in the ACCM/SCCM1991 Chicago conference. Forty cases of healthy persons were chosen as control group （ group C）. The test results of P-selectin and platelet count were compared among the three groups, then the test result of P-selectin and platelet count between sur- vival group and the death group according to the prognosis of group A were compared. Results The plasma level of P -selectin of group A was higher than that of group B and group C, but the platelet count was lower, the difference was significant. The level of P -selectin of the death group was significantly higher than that of the survival group, but the platelet count was significantly lower. Conclusions The test result of P-selectin and platelet count can be used as important indicators to evaluate the prognosis and severity of the patients with severe infection.Prognosis
