P57,P53,bcl-2,ki-67联合应用在葡萄胎诊断中的意义

摘　　要：目的:研究p57,P53,bcl-2,ki-67蛋白表达在完全性葡萄胎(CM)、部分性葡萄胎(PM)及水肿流产胎(HA)的诊断及鉴别诊断意义。方法:对21例完全性葡萄胎,28例部分性葡萄胎,18例水肿流产胎的标本进行4种抗体免疫组化分析,同时选取10例正常胎盘组织作为对照。结果:在CM中,p57表达不明显,而P53,bcl-2,Ki-67表达增强;在PM,HA及对照组中p57呈现高表达,P53,bcl-2,Ki-67则表达较弱。这四种抗体的表达在完全性葡萄胎与部分性葡萄胎、水肿流产胎及对照组之间的差异具有显著性(P<0.01),而部分性葡萄胎与水肿流产胎、水肿流产胎与对照组之间差异不显著(P>0.05)。结论:p57和Ki-67表达对CM,PM和HA的鉴别诊断有重要作用,而P53,bcl-2表达对判断预后及预测恶变风险有意义。Objoetive： To study the significance of p57,P53,bcl-2,ki-67 applied to the diagnosis on the cases of complete mole （CM）, partial mole （PM）, and hydropic abortion（HA）. Methods： The immunohistochemical expression of the 4 types protein was investigated by using paraffin-embedded tissue sections in histologically unequivocal cases of CM （n=21）, PM（n=28）, HA（n=I 8）, and selected 10 normal placentas as the control group. Results： In the CMs, expression of p57 was absent or weak, expression of Ki-67, P53, bcl-2 were strong In contrast, P57 was strongly and continuously expressed in PMs, but expression of P53, bcl-2, Ki-67 were weak. The difference of P57, P53, bel-2, Ki-67 expression between CMs and PMs, HA, normal control group was statistically significant （P〈0.01）, but there was no statistically significant difference between PMs and HAs （P〉0.05）. Conclusions： p57 and Ki-67 immtmostaining results correlate well with morphological features of molar Pregnancies and are helpful in differential diagnosis of CM from PM and HA, the expression of P53, bcl-2 was important for prognosis and malignant transformation of risk.
