双侧子宫动脉栓塞化疗术治疗围绝经期功血36例临床分析

摘　　要：目的探讨双侧子宫动脉栓塞化疗术治疗围绝经期功血的可行性。方法 36例围绝经期功血并发不同程度贫血患者采用经股动脉穿刺插管至双侧子宫动脉行栓塞化疗。结果 36例患者均成功完成双侧子宫动脉栓塞化疗,术后无严重并发症发生,术后46～85d后月经来潮,内膜活检为分泌期改变,经量明显减少,总量约40～70ml,随访6～18个月月经周期恢复正常,均未再次出现功血。结论双侧子宫动脉栓塞化疗术在围绝经期功血治疗上提供了一条安全、创伤小、保留女性生殖系统完整性及维持盆底结构稳定性、同时不影响卵巢功能、避免长期药物治疗后副反应的可靠途径。Objective To investigate the feasibility of bilateral uterine artery embohzation chemothera- py in the treatment of perimenopause dysfunctional uterine bleeding（DUB）. Methods 36 cases of DUB Com- plicated with different degree of anemia undergoing percutaneous femoral and take bilateral uterine artery embo- lization chemotherapy. Results 36 patients were successfully completed bilateral uterine artery embolization chemotherapy, there were no serious complications after operation, menorrhea was came after 46 to 85 days, en- dometrial biopsy showed the secretory phase was changed, the menstrual obviously decreased, a total of about 40 toT0 ml, all patients were followed up for 6 to18 months and return to normal menstrual cycle, and the DUB was not found again after treatment. Conclusion Bilateral uterine artery embolization chemotherapy was a reliable way to treats with DUB. It＇s safety and minimal invasive,it can reserved the integrity of female reproductive system, maintained the stability of pelvic floor structure, and avoid reaction after long-term drug therapy.
