参麦注射液对食管癌开胸患者术后肺换气功能的影响

摘　　要：目的探讨胸外科患者围术期静脉滴注参麦注射液对患者术后肺功能恢复的影响。方法 50例食管中段癌患者,单盲法随机分为参麦治疗组(SM,n=26),对照组(C,n=24)。SM组在麻醉诱导前、术毕30 min内将参麦注射液0.6 ml/kg加入100 ml生理盐水静脉滴注完毕各一次,C组则在相同时间内静脉滴注等量生理盐水。在诱导前(T0)、术后2 h(T1)、术后6 h(T2)、24 h(T3)各时点行血气分析。结果与T0时比较,两组术后6 h PaO2明显下降(P<0.05)、PA-aDO2和RI明显升高(P<0.05),术后24 h PA-aDO2和RI也明显升高(P<0.05);与C组比较,SM组术后6 h PaO2明显升高(P<0.05),PA-aDO2、RI明显降低(P<0.05)。结论食管癌开胸患者术后换气功能有一定程度损伤,参麦注射液确实有改善肺的换气功能,达到一定的肺保护作用。Objective To investigate the effect of Shenmai injection on pulmonary gas exchange function in esophageal carcinoma patients of thoracic surgery. Methods Fifty esophageal carcinoma patients were randomly divided into two groups ： Shenmai injection group （ SM, n = 26） and control group （ C, n = 24）. Patients in group SM were injected Shenmai injection 0.6 ml/kg and physiological saline 100 ml, while patients in group C were injected equal volume of normal saline instead 15 rain before anesthesia induction and after the completion of operation within 30 minutes. Blood samples which were used for blood gas analysis were taken before anesthesia induction （ To, baseline） and 2 h （T1 ） ,6 h （T2 ）, 24 h （T3 ） after operation. Results Compared with the baseline values at To, PaO2 was significantly decreased （P 〈 0.05 or 0.01 ）, PA-aDO2 and RI were significantly increased （P 〈 0.05 or 0.01 ） at T2 in 2 groups, PA-aDO2 and RI were significantly increased （P 〈 0.05 or 0.01 ） at T3 in 2 groups. Compared with group C, the changes of PaO2,PA-aD02 and RI were significantly mitigated in group SM （P 〈 0.05 ）. Conclusions The thoracic esophageal carcinoma surgery could induce pulmonary gas exchange impairment and Shenmai injection had pulmonary protection by improving pulmonary gas exchange impairment.
