髂静脉狭窄综合征合并下肢静脉曲张的一站式介入治疗(附15例报告)

摘　　要：目的探讨髂静脉狭窄综合征合并下肢静脉曲张一站式介入治疗的临床价值。方法 2012年1月至11月,我院共收治髂总静脉受压综合征合并下肢浅静脉曲张患者15例。先采用小切口高位游离大隐静脉,在介入下以此为入路穿刺处理髂静脉狭窄,然后同期行高位结扎加导管引导泡沫硬化注射治疗下肢静脉曲张。结果所有患者治疗效果良好,髂静脉受压3例单纯行球囊扩张,12例行球囊扩张+支架植入术,术后给予规则抗凝治疗,无一例死亡和肺动脉栓塞。结论髂静脉狭窄综合征合并下肢静脉曲张的一站式介入治疗近期效果良好,微创,并发症少,操作较简单。Objective To evaluate the one-stop interventional treatment of iliac vein stenosis syndrome complicated by lower extremity varicose veins in clinic. Methods From Jan. 2012 to Nov. 2012, a total of 15 patients with iliac vein stenosis complicated by lower extremity varicose veins, who were treated in our department, were included in the present study. All pa- tients received endovascular therapy in iliac vein compression via great saphenous which was high dissociated first through small incision, and high ligation and foam sclerotherapy under the guidance of catheter was used in the treatment of varicose veins in the same period. Results All patients recovered well. Three patients received percutaneous transluminal angioplasty alone; Twelve patients received balloon dilatationwas and stent placement. Regular anticoagulant therapy was given after intervention. No death or pulmonary embolism occurred. Conclusion One-stop interventional treatment on iliae vein stenosis syndrome com- plicated by lower extremity varicose veins is effective, with minimally invasion, few complications, simple operation.
