老年慢性阻塞性肺病及其合并院内感染或糖尿病患者免疫功能的研究

摘　　要：目的　探讨老年慢性阻塞性肺病 (COPD)及其合并院内感染 (HI)或糖尿病 (DM)患者免疫功能的变异情况 ,为临床治疗提供一定的依据。方法　选择老年 COPD35例 ,COPD合并 HI12例 ,合并 DM2 5例 ,检测其外周血 T淋巴细胞亚群 (TL S)水平和 Ig G、 Ig A、 Ig M的含量 ,并与健康人比较。结果　各组 CD3、 CD4和 CD4/ CD8比值均显著低于对照组 (P <0 .0 1) ,COPD合并 HI组 CD3、CD4明显低于 COPD组及 COPD合并 DM组 (P <0 .0 5 ) ,各组 Ig G均显著低于正常对照组 (P <0 .0 1、P <0 .0 5 ) ,COPD组、COPD合并 DM组 Ig A以及 COPD组、COPD合并 HI组 Ig M均低于正常对照组 (P <0 .0 5 )。结论　各组患者均存在显著的免疫功能紊乱 ,在治疗的同时应采用有效方法提高机体细胞免疫和体液免疫水平 ,对维持免疫稳定和提高疗效具有重要意义。Objective To investigate the immune functions in old patients of COPD with and without hospital infection (HI) or diabetes mellitus (DM),and to provide certain relevant information for clinical therapy Methods\ We examined the level of T Lymphocyte Subsets (TLS) and IgG,IgA,IgM in peripheral blood from 35 cases COPD,12 cases COPD with HI,and 25 cases COPD with DM,then compared with 18 healthy donors Results CD\-3、CD\-4 and ratio of CD\-4/CD\-8 in every group were significant lower than that of control group (P<0 01),CD\-3、CD\-4 of COPD with HI were significant lower than COPD and COPD with DM (P<0 05) IgG of every proup were significant lower than that of control group (P<0 01,P<0 05);IgA of COPD and COPD with DM,IgM of COPD and COPD with HI all were significant lower than that of control group (P<0 05) Conclusion\ Every group all exist immune function disorder We should improve the immunity of patients in the process of therapy.It has important significance to support immune function and increase curative effect
