摘 要:目的检测系统性红斑狼疮(SLE)患者血清B细胞活化因子(BAFF)、干扰素α(IFN-α)表达水平,探讨BAFF、IFN-α与SLE疾病活动指标(SLEDAI)的关系及BAFF与IFN-α的相互关系。方法采用酶联免疫吸附法(ELISA)检测60例SLE患者及20例健康对照血清BAFF、IFN-α水平。结果 SLE患者活动组血清中BAFF水平显著高于非活动组及健康对照组,(1 141.81±650.12)ng/L vs(652.87±414.94)ng/L、(573.77±241.97)ng/L(均P<0.01)。SLE患者活动组血清中IFN-α水平显著高于非活动组及健康对照组,5.81(9.91)ng/L vs 4.41(4.27)ng/L,3.89(1.18)ng/L(均P<0.01)。SLE患者血清中BAFF水平与SLEDAI、血沉(ESR)呈正相关(r=0.671、0.278,P<0.05或<0.01);与补体(C3)呈负相关(r=-0.320,P<0.05)。SLE患者血清中IFN-α水平与SLEDAI、ESR、C反应蛋白(CRP)呈正相关(r=0.426、0.398、0.300,P<0.05或<0.01);与C3呈负相关(r=-0.297,P<0.01)。SLE患者中32例抗ds-DNA抗体阳性者血清BAFF、IFN-α水平高于28例阴性者(1 027.45±587.32)ng/L vs(666.06±379.10)ng/L(P<0.01);6.85(13.61)vs 4.03(1.54)ng/L(P<0.05)。SLE患者血清中BAFF与IFN-α呈正相关(r=0.290,P<0.01)。结论SLE患者BAFF、IFN-α水平明显升高,并与SLEDAI相关;BAFF和IFN-α存在正相关,提示在SLE发病中可能起相互调控的作用。Objective To detect the levels and clinical significance of B cell activating factor (BAFF) and interferon αa(IFN-α) in systemic lupus erythematosus(SLE) ,and to analyze the correlation between IFN-α and EAFF as well as SLE disease activity index(SLEDAI). Methods Serum EAFF and IFN-α levels were measured by enzyme- linked immunosorbent assay(ELISA) in 60 patients with SLE and 20 health controls. Results SLE patients with active disease had higher EAFF levels than the patients with inactive disease and health controls, (1 141.81 ± 650.12)ng/L vs (652.87±414.94) ng/L vs (573.77± 241.97) ng/L (all P 〈0.01). SLE patients with active disease had higher IFN-α levels than the patients with inactive disease and health controls,5.81(9.91)ng/L vs 4.41(4.27) ng/L vs 3.89 (1.18) ng/L (all P d0.01). BAFF level was positively correlated with SLEDAI ( r =0. 671, P d0.01) ,erythrocyte sedimentation rate(ESR) ( r =0. 278, P d0.05) and negatively correlated with C3 compliment( r = --0. 320, P 0.05) in SLE patients. IFN-a level was positively correlated with SLEDAI ( r =0. 426, P 〈0.01) ,ESR( r =0. 398, P〈 0.01) ,CRP( r =0. 300, P d0.05) and negatively correlated with C3 ( r = -0. 297, P 〈0. 01). 32 patients with positive anti-dsDNA antibody had significantly higher levels of BAFF and IFN-α than anti-dsDNA antibody negative patients(1 027.45±587.32) ng/L vs (666.06±379.10) ng/L( P 〈0.01) and 6.85(13.61) vs 4.03(1.54) ng/L( P 0.05). BAFF level was positively correlated with IFN -α in SLE patients( r =0. 290, P 〈0.01). Conclusion BAFF and IFN-α are associated with SLEDAI, and EAFF level is positively correlated with IFN-α in SLE patients, suggesting EAFF and IFN-α may interact in the course of SLE.
