三种心脏负荷试验对冠心病诊断价值的比较

摘　　要：80例可疑冠心病心绞痛病人 ,分别接受多巴酚丁胺负荷超声心动图、Master二级梯运动心电图和超速食道心房起搏试验 ,比较三种心脏负荷试验诊断冠心病的敏感性、特异性和准确性。结果显示 :多巴酚丁胺超声心动图诊断冠心病的敏感性高于后两种试验 (86 .8VS 6 7.9% ,P <0 .0 5 ;86 .8US6 0 .4% ,P <0 .0 0 5 )。多巴酚丁胺和心房起搏试验的特异性高于运动负荷试验 (85 .2US 5 5 .6 % ,P <0 .0 5 ;81.5VS 5 5 .6 % ,P <0 .0 5 )。多巴酚丁胺试验的诊断准确性明显高于运动负荷试验 (92 .0VS 75 .0 % ,P <0 .0 5 )。结论To comparing the sensitivity, specificity, accuracy of three kinds of loading test in diagnosis of coronary heart disease(CAD). 80 patients with suspected coronary heart disease and angina pectoris (53 cases with CAD and 27 cases without significant CAD) accepted dobutamine loading test, Master“2-step” exercise test and auricular pacemaking. The sensitivity of dobutamine loading echocardiograph was higher than the others (86.8 VS 67.9%, P<0.05; 86.8 VS 60.4%, P<0.005). The specificity of dobutamine Loading test and auricular pacemaking was higher than exercise test (85.2 VS 55.6%, P<0.05; 81.5 VS 55.6%, P<0.05). The accuracy of dobutamine echocardiography was also higher than that of excercise test (92.0 VS 75.0%, P<0.05). Thus, dobutamine echocardiography may be superior to two-step excercise test and auricular pacemaking.
