加压锁定钢板治疗浮肘损伤

摘　　要：目的探讨浮肘损伤的机制及加压锁定钢板的疗效。方法自2005年5月至2011年6月采用加压锁定钢板治疗浮肘损伤24例,其中男16例,女8例;年龄18～61岁,平均34.6岁;闭合性骨折12例,开放性骨折12例(GustiloⅡ型7例,GustiloⅠ型3例,GustiloⅢ型2例)。结果 24例获得6个月～3年随访,平均18.4个月。术后3～6个月X线片显示骨折愈合,未发生骨折延迟愈合或不愈合。根据Khalfayan上肢功能评分,优10例,良10例,中2例,差2例,平均84.7分(54～96分),优良率83.3%。12例合并桡神经损伤,其中8例神经功能完全恢复,4例残留部分感觉或运动功能障碍。2例臂丛神经损伤二期行手术治疗,功能部分恢复。结论加压锁定钢板治疗浮肘损伤具有固定牢靠、术后可早期功能锻炼、并发症少等优点,是治疗浮肘损伤的一种有效方法。Objective To study the mechanism of floating elbow and the clinical effects of locking compression plate in the management of floating elbow. Methods From May 2005 to July 2011,24 cases of floating elbow were treated with locking compression plate after open reduction. There were 12 cases of closed fracture and 12 cases of open fracture including Gustilo Ⅱ type 7 cases,Gustilo I type 3 cases,Gustilo Ⅲ type 2 cases. Results All cases were followed up for 18.3 months （range,6~36 months）. According to elbow score based on a 100-point system as de- scribed by Khalfayan,there were excellent in 10 cases,good in 10 cases,fair in 2 cases and poor in 2 cases. The mean score was 84.7 （range, 96~ 54）,and the excellent and good rate was 83.3 %. There was no nonunion delayed-union or malunion. 8 of 12 cases with radial nerve palsy gained full recovery,other 4 cases gained partial recovery. 2 cases with brachial plexus palsy accepted surgery treatment later and the fuction partially recovered. Conclusion Locking com- pression plate osteosynthesis is an effective treatment for patients with floating elbow.
