摘 要:目的:研究小儿肺炎及其不同病原体感染中血清可溶性髓系细胞触发受体-1(sTREM-1)水平的变化情况。方法:随机选取重症肺炎及普通肺炎患儿各30例,分别检测刚入院及治疗5d后的血清sTREM-1水平,并检测其具体病原体感染情况。结果:在治疗前,重症肺炎组与普通肺炎组血清sTREM-1水平显著高于正常对照组,而重症肺炎组血清sTREM-1水平与普通肺炎组比较差异无统计学意义。治疗后,重症肺炎组的血清sTREM-1水平仍显著高于普通肺炎组和正常对照组(P<0.05),但普通肺炎组与正常对照组比较,差异无统计学意义(P>0.05)。重症感染组与普通感染组均以细菌感染最为常见,而且两组的单独细菌感染及混合感染的血清sTREM-1水平均显著高于非细菌感染者。结论:血清sTREM-1水平是小儿肺炎的敏感指标,且与炎症轻重相关。而细菌感染是sTREM-1增高的主要因素,但在小儿肺炎中,非细菌感染亦可引起sTREM-1增高。Objective:To study the changes of serum soluble myeloid cells trigger receptor 1 (sTREM-1) level in different type of pathogens infection of the pediatric pneumonia. Methods :Randomly selected 30 cases of severe pneu- monia and 30 cases of common pneumonia respectively. The serum sTREM-1 level before treatment and 5 days treat- ment after were detected. And which type of pathogens infection were detected too. Results :Before treatment ,serum sTREM-1 level of severe pneumonia group and common pneumonia group were significantly higher than that of nor- mal control group ,and there was significantly different of serum sTREM-1 level between the severe pneumonia group and the common pneumonia. After treatment,serum Strem-1 level of the severe pneumonia group is still significantly higher than that of common pneumonia group and normal control group, but there was no significantly different be- tween the common pneumonia group and the normal control group. And the bacterial infections are most in the severe pneumonia group and the common pneumonia group, serum sTREM-1 level of The individual bacterial infection and mixed infection of of 2 groups is significantly higher than that of the no-bacterial infection groups. Conclusion :Serum sTREM-1 level is a sensitive indicators of pediatric pneumonia,and it correlates closely with the severity 6f the dis- ease. And bacterial infection is the common factor which increases serum sTREM-1 level. But the non-bacterial infec- tion can also cause serum sTREM-1 level increased in pediatric pneumonia.
