腰神经根管狭窄症的治疗

摘　　要：目的 探讨神经根管减压术治疗神经根管狭窄症的疗效.方法 回顾性分析2003 年1 月~ 2011 年12 月本院收治的23 例神经根管狭窄症患者,均给予神经根管减压术治疗,观察患者术前和术后3、6、12 个月的VAS 评分和JOA 评分情况,并进行影像学评价.结果 23 例患者的术后VAS 评分与术前相比明显降低,差异有统计学意义（P ＜ 0.05）,而且随时间增加VAS 评分明显降低;患者术后的JOA 评分比术前明显增加,但术后各时段的JOA 评分间差异无统计学意义（P ＞ 0.05）.结论 神经根管减压术可对狭窄的神经根管进行充分减压,应用椎弓根钉棒系统进行植骨内固定,可有效维持手术部位的稳定性,术后疗效确切,是一种治疗神经根管狭窄症的有效方法.Objective To probe into the effect of decompression of nerve root canal on straitness of nerve root canal. Methods Clinical data of 23 patients with straitness of nerve root canal received by our hospital from Jan. 2003 to Dec. 2011 were analyzed retrospectively. The decompression of nerve root canal was administered to all patients. VAS scores and JOA scores of all patients before decompression operation and at 3,6 and 12 months after decompression operation were observed. And,imaging evaluation was made. Results The postoperative VAS scores of 23 patients were obviously lower than those before decompression treatment（P 〈 0.05）. There was a significant difference. And VAS scores declined obviously with the increase of time. In addition,the postoperative JOA scores of patients were higher than those before decompression treatment. However,the differences between the postoperative JOA scores in all periods of time had no statistical significance（P ~ 0.05）. Conclusion The decompression of nerve root canal can make full decompression on narrow nerve root canal. Atlantoaxial pedicle screw-rod system for bone graft and internal fixation can effectively maintain the stability of the operative site and has excellent postoperative efficacy. Therefore,it is an effective method for treating straitness of nerve root canal.
